Schedule: Friday–Sunday 9:00 a.m. – 7:00 p.m.

Location: Muskegon, MI

We are seeking a mission-driven Advanced Practice Provider committed to promoting health, preventing disease and improving lives.

Position Summary:

The Advanced Practice Provider delivers high-quality medical care in accordance with accepted professional standards. Responsibilities include patient evaluation, treatment, referrals, and thorough documentation. This role supports both scheduled and walk-in primary care services.

Key Responsibilities:

Serve as an active member of the care management team, providing leadership and clinical guidance
Deliver comprehensive primary care within scope of PA/NP training
Coordinate and support preventive care, diagnosis, treatment, and follow-up services
Communicate patient care plans effectively with team members
Prescribe medications as appropriate
Maintain accurate and timely documentation of diagnoses, treatments, medications, and consultations
