We are seeking a dedicated and mission-aligned Advanced Practice Provider to join our integrated care team. The Advanced Practice Provider will provide high-quality, age-appropriate, and specialty-specific care to LLCHC outpatients in coordination with a collaborating physician. The APP will practice within their scope of licensure, certification, and education, and will work alongside a multidisciplinary team to deliver holistic care in alignment with LLCHC’s faith-based mission.

- Deliver high-quality, patient-centered primary care to individuals and families of all ages.
- Manage clinical schedules efficiently while maintaining patient access and continuity of care.
- Utilize evidence-based guidelines to support diagnostic and treatment decisions.
- Document care in a timely, accurate, and compliant manner within the EHR (Epic experience preferred).
- Serve as a clinical leader in your care team and actively participate in team-based care delivery.

Work Environment & Physical Requirements
- Primarily clinic-based; may include occasional community outreach or telehealth.
- Ability to lift up to 25 lbs, bend, stoop, and move intermittently throughout the day.
- May require occasional evening or weekend hours based on patient needs.
- Medical care will be of the highest quality and will be monitored by peer review and continuous quality improvement.
- Master's or Doctoral degree in Nursing or Physician Assistant Studies
- Board certification in the relevant specialty
- 2+ years of clinical experience in a healthcare setting
- Strong communication and interpersonal skills
- Experience with telemedicine services
- Current and unrestricted state license to practice as an NP or PA
- National certification from recognized certifying bodies (e.g., AANP, ANCC, NCCPA)
- Current BLS certification (ACLS and PALS preferred).
- Demonstrated commitment to serving diverse, underserved, or vulnerable populations.
- Strong interpersonal and communication skills with a team-oriented approach.
- Prior experience in a community health center or similar primary care setting.
- Familiarity with EPIC or other EHR systems.
- Bilingual (Spanish, Somali, or Arabic) is a plus.
