Your Role
A registered Physician Assistant with advanced skills in the assessment of physical and psychosocial health and illness of patients and their families. The candidate will be assigned to work in conjunction with all providers of each medical department functioning in a self-directed and motivated manner as a member of the medical team. Additionally, skills in community assessment, especially in identifying and interacting with at risk populations desired. Determines and provides data for appropriate clinical manager and providers.
Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts
Qualifications
Education: Graduate of an Accredited School of Physician Assistants with Master’s
Level or equivalent desired. Successful completion of an organized
program of study meeting CAAHEP guidelines for PA program required.
Required Licensure and/or Certification: Certified and Licensed as a Physician Assistant in the State of Illinois. ASCP preferred.
Minimum Experience: Experience in working with adult, pediatric and women’s health populations in an independent setting managing a wide range of medical problems. Experience in prioritizing & planning (triage) and in educational and outreach activities.
Physical Environment: Exposed to infectious disease, unpleasant odors and fumes. The use of Universal Precautions is mandatory.

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
