PRIMARY FUNCTION:

The Physician is responsible for the delivery of patient care services, including assessment, diagnosis, treatment planning and coordination, monitoring, and evaluation of medical care to individual patients in a variety of setting including clinic, hospital and nursing facilities.

PRINCIPLE DUTIES:

1. Provides direct physician services to patients including examination, diagnosis, treatment planning and coordination, and evaluation of progress. Reviews and follows up on all reports of ancillary services provided to patients.
2. Provides outpatient, inpatient and extended care services to patients.
3. Arranges for appropriate coverage of his/her patients when unavailable.
4. Uses SOAP format for recording patient contacts and maintains problem-oriented clinical records. Keeps records that are adequately detailed, truthful, clear and accurate to allow good continuity of patient care and accurate billing (coding of diagnoses, services provided, and charges). The physician’s clinical notations should facilitate another provider’s assuming follow-up care. Assures a summary of services given, generally referred to as “health maintenance”, is kept up-to-date in individual patient records. Notations should be completed on the day that services are provided. Dictated notes should be read, corrected and signed or initialed by the provider in accordance with clinic protocol.
5. Provides billing information relative to all billable services, i.e., completed encounter forms and hospital charge sheets.
6. Demonstrates appropriate work habits which include:
7. Arriving on time for clinical sessions and/or meetings – unless detained by an emergency.
8. Communicates appropriately with patients and other providers and returns patient/provider phone calls as promptly as possible:
9. Utilizing appropriate infection control measures;
10. Maintaining appropriate patient confidentiality.
11. Shares call coverage for Health Help patients.
12. Cooperates in promoting the team concept of health care delivery. Available for phone consultation for Health Help’s mid-level and ancillary staff as needed.
13. Maintains Kentucky and specialty specific requirements of approved continuing medical education and secures Clinic Director’s approval of CE reimbursed by Health Help within defined benefit parameters.
14. Collaborate with Medical Director and other provider staff members in development and periodic updating of standards of care, principles of practice, standing orders and/or clinical protocols (including those governing mid-level providers).
15. As requested, participates with professional and allied health training programs in student and resident rotations.
16. Assists in recruitment and orientation of new staff.
17. Assists in health care planning. Maintains familiarity with Health Help’s mission, goals, and objectives and works with Medical Director and staff to assure these are met. Helps assure clinic productivity, efficiency and financial objectives are met.
18. As assigned, attends meetings or makes presentations to community groups, associations or other health organizations as Health Help’s representative.
19. Attends Health Help staff meetings as requested.
20. Maintains unrestricted Kentucky Medical License, DEA certification, and privileges to appropriate hospitals and long-term care facilities.
21. Performs related work and responsibilities as assigned.
22. Participates in fulfilling Health Help’s mission in meeting the health needs of the community.

JOB REQUIREMENTS:

Minimum Education: Board Certified/Board Eligible M.D.; D.O.

Required License: An unrestricted Kentucky Medical License.

Accountable to the Chief Medical Officer, and ultimately the Chief Executive Officer.

TYPICAL WORKING CONDITIONS:

A position exempt from wage and hour laws; requires hours in excess of 40 per week including night and holiday work from time to time. Work is performed in health care environments including, medical office, hospital and nursing home with exposure to communicable diseases. Position may include possible exposure to aggressive or violent patients, and requires the ability to work under stressful conditions, often for long periods of time.

Our Evening & Weekend access schedule creates an alternative work schedule which allows for 4-day weekends every other week, while extending access to care for health center patients. This Primary Care APRN position cares for patients in the Berea Primary Care Clinic located inside CHI - St. Joseph - Berea.

BASIC FUNCTION

Nurse Practitioner provides primary care services, including assessing and managing patients in the clinic, following established standards and practices.

PRINCIPLE DUTIES

1. Performs a complete physical exam, obtains complete medical history and records findings. Orders appropriate laboratory and diagnostic procedures. Interprets and integrates data to determine preliminary diagnosis and therapeutic plan.
2. Conducts one on one patient teaching and education. Educates patients regarding health and illness prevention.
3. Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
4. Participates in Quality Assurance activities and chart reviews.
5. Records physical findings, and formulates plan and prognosis, based on patient’s condition. NP agrees to keep current and accurate records in accordance with clinic medical protocol and is obligated to provide adequately detailed information to allow accurate and truthful coding of diagnosis and services provided.
6. Discusses case with Physician and other health professionals, when/as needed to prepare comprehensive patient care plan.
7. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
8. Refers patients to Physician for consultation or to specialized health resources for treatment.
9. Administers medications and injections in compliance with state law and clinic guidelines. Sutures minor lacerations.
10. Instructs patient/family regarding medications and treatments.
11. Recommends community resources to meet patient and family needs.
12. Collaborates with physicians in managing acute and long-term medical needs of patients. Provides monitoring and continuity of care between physician visits. Maintains and reviews patient records, charts and other pertinent information. Posts tests and examination results. Triages patient telephone calls and provides consultation.
13. Maintains patient confidentiality.
14. Performs other duties as assigned.

ACCOUNTABILITY

Reports to the Chief Medical Officer.

SUPERVISORY RESPONSIBILITIES

Nurse Practitioner may supervise as preceptor to a nurse practitioner student per organizational guidelines. Employee carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include instructing students; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.

TYPICAL PHYSICAL DEMANDS

May require prolonged sitting, some bending, stooping, stretching, and lifting (generally not in excess of 50 pounds at a time). Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment. Requires normal range of hearing and eyesight to record, prepare, and communicate appropriate reports.

TYPICAL WORKING CONDITIONS

Work is performed in a medical or dental business office environment. Requires frequent contact with staff and the public. Frequently exposed to blood, toxic or caustic chemicals. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to a clinic environment. Work may be stressful at time.

WORKING HOURS

40+ hours/week. A position exempt from wage and hour laws, job may require work weeks in excess of forty hours and/or irregular schedule from time to time.

SALARY

Starting at $98,000, annually.
