Position Summary:
Develops and implements individualized dental care plans for patients; performs patient education, discharge planning, and patient/family teaching under the supervision of a qualified dentist.
Essential duties and Responsibilities:
· Assess dental condition and needs of the patient; use screening procedures, to include medical history review, dental charting, and periodontal charting; takes patients vital signs as required.
· Delivers direct patient care to patients as assigned using established dental hygienist procedures and performs routine treatment procedures.

· Assists with or institutes emergency measures for sudden adverse developments during treatment of patients.

The Nurse Practitioner may be directed by the clinical Director to perform other duties in accordance with funding policies, procedures, and protocols.
Essential duties and Responsibilities:
· Serve as a member of the Center Street Community Health Center health care team establishing a positive, professional relationship with collaborating physician and other members of the health care team.
· Perform routine physical examinations and preventative health measures including, but not limited to immunizations, developmental screening, and routing laboratory screening/procedures appropriate to gender and age group.
· Diagnosis and treat common and uncomplicated health conditions. Collaborate, consult and refer any patient who has complicated or uncommon health problems to other physicians in the health center or to the community as appropriate.
· Prescribe medications and devices incidental to the provision of routine health care, family planning and perinatal care for essentially healthy patients in accordance with the State of Ohio law.
· Chart in the medical record utilizing age-appropriate wellness assessment tools or problem oriented medical record system, assuring accurate and timely completion of data entry into the patient medical record.
· Institute emergency medical measures in life-threatening situations. Provides first aid as needed.
· Provide precise information and education about diagnosis to the patient to enable informed decision making and provide written follow-up. Include family members according to patient instructions.
· Incorporate preventative health care aimed at improving and maintaining health status based upon individual, cultural, sociological, psychological and environmental factors.
· Coordinate patient services and referrals to promote and maintain continuous comprehensive care at entry level.
· Provide support and counseling for patients with stable chronic disease.
· Refer any patient with unstable chronic disease to a physician in the health center or community as appropriate.
· Follow universal precautions in all activities.
· Utilize supplies in a cost-effective manner.
· Perform clinical and outreach related tasks.
· Participates in Quality Assurance/ Quality Improvement activities and other initiatives as appropriate.
· Adhere to applicable laws and regulations.

· Collaborate with administrative and physician leadership to develop and implement pharmacist-driven clinical and dispensing services for the patient population served.
· Develop, implement, and maintain collaborative practice agreements to improve medication adherence among high-risk patients and support provider satisfaction.
· Implement best practices and ensure departmental compliance with all 340B program rules and regulations.
· Ensure adequate staffing levels to meet the operational and clinical needs of the pharmacy.
· Maintain accurate and complete records of all prescription medications received, stored, and dispensed.
· Establish and enforce policies and procedures to ensure the accurate dispensing and proper labeling of prescriptions.
· Maintain the security of the prescription area and its contents at all times, including restricting access to authorized personnel only and reporting any theft, loss, or diversion of controlled substances to the appropriate state and federal agencies in accordance with applicable regulations.
· Ensure that professional consultation with patients and providers is provided only by licensed pharmacists or interns/externs under the pharmacist’s immediate personal supervision.
· Oversee and maintain the Pharmacy’s Continuous Quality Improvement (CQI) Program to reduce medication errors and promote patient safety.
· Ensure that misbranded, deteriorated, adulterated, improperly stored, outdated, or sample medications are not dispensed or maintained in active pharmacy stock.
· Maintain the prescription area in a clean, orderly, and sanitary condition.
· Serve as Pharmacist-in-Charge in accordance with Ohio pharmacy laws and regulations.
