Physician Assistant - Family

Under the direction of the Medical Director, the Nurse Practitioner (APRN) conducts History, Examination and Treatment of simple to complex medical conditions whereby the conditions are typically managed on an outpatient basis within the SCA scope of practice. Examples of such conditions include: acute upper respiratory viral infectious conditions, acute ENT viral and bacterial infectious conditions (e.g., otitis externa, otitis media, and pharyngitis), mild to moderate asthma exacerbations, simple dermatological disorders (e.g., uncomplicated burns, contact dermatitis, simple I&D, and atopic dermatitis) and common sexually transmitted diseases.

Duties: Under the supervision of the Dental Director, the Dentist is responsible for providing clinical dental services and preventative dental care in an out-patient clinic or at one of our many community sites. Must be cognizant of the public health, social, and economic factors which impact the overall health of NEON's patients. Able to creatively work with other health care professionals.

Dental Hygienist cleans calcareous deposits, accretions, and stains from teeth and beneath margins of gums; Applies fluorides and other cavity preventing agents to arrest dental decay. Examines gums, using probes, to locate periodontal recessed gums and signs of gum disease. Performs x-ray and makes impressions for study casts when necessary; documents conditions of decay and disease for diagnosis and treatment by dentist in EHR system; Educates patient in maintaining good oral health and develops and promotes community dental health programs.

Adult, Child & Family Therapist/Chemical Dependency Counselor provides behavioral health services to NEON patients experiencing mental health conditions and chemical dependency. Conducts history and assessment; determines diagnosis; develops treatment plan and provides clinical counseling, psychosocial interventions and social psychotherapy. Provides Screening, Brief Intervention, Referral and Treatment (SBIRT), Substance Abuse Therapy and Medication Assisted Treatment (MAT). Coordinates the Substance Abuse Expansion Program services and staff. Provides supervision of Chemical Dependency Counselors and coordinates integration of Primary Care and Behavioral Health Substance Abuse care team. Provides supervision of Chemical Dependency Counselors and students with required supervisory licensure. May provide training and supervision to students within the scope of supervisory licensure.

Northeast Ohio Neighborhood Health Services, Inc. (NEON) is a FQHC, Federally Qualified Health Center (FQHC). NEON offers multi-specialty community-based, Primary Care across the lifespan with Internal Medicine, Family Medicine, Pediatrics, OB/Gyn and Behavioral Health Care providers. General Duties

Under the general supervision of the Medical Director, the Pediatrician is responsible for providing and overseeing the healthcare of babies, children and adolescents.

Pediatrician must have general medical knowledge and an understanding of how treatments affect different developmental growth stages. Provide primary care to patients that involves the following: history, examination and treatment of simple to moderately complex medical conditions affecting patients, whereby the conditions are typically managed without specialty or subspecialty consultant collaboration on an outpatient basis. Work within his/her scope of training and that established by the Medical Director and when the needs of the patient fall outside that scope, refer the patient to other healthcare providers whose scope include that of serving the patient.

Provides behavioral health services to NEON patients experiencing mental health conditions and emotional disorders. Conducts history and assessment; determines diagnosis; develops treatment plan and provides clinical counseling, psychosocial interventions and social psychotherapy. Provides supervision of Social Workers and coordinates integration of Primary Care and Behavioral Health care teams.

General Duties

Under the general direction of the Medical Director, the Physician Assistant conducts History, Examination and Treatment of simple to complex medical conditions affecting patients across the lifespan whereby the conditions are typically managed with or without specialty or subspecialty consultant collaboration (e.g., oncology disorders, sickle cell disease, severe coronary artery disease, severe and complicated hypertensive disorders, conditions requiring anticoagulation, and HIV). Conditions are managed under the supervision, control, and direction of an Internal Medicine or Family Medicine Physician with whom the physician assistant has a supervision agreement approved by the Ohio Medical Board.

Performs restorative procedures such as fillings, crowns, root canal treatments, extractions and fixed and removable prosthodontics. Must be cognizant of the public health, social and economic factors that impact the oral and overall health of patients in an underserved setting.

Under the general direction of the Medical Director, the Family (FNP) and Adult (ANP) Nurse Practitioners conduct History, Examination and Treatment of simple to complex medical conditions affecting patients within their scope of practice whereby the conditions are typically managed with or without specialty or subspecialty consultant collaboration (e.g. oncology disorders, sickle cell disease, severe coronary artery disease, severe and complicated hypertensive disorders, conditions requiring anticoagulation, and HIV). Conditions can be managed with our without the director involvement of an Internal Medicine or Family Medicine physician under current Standard Care Arrangement.
