Nurse Practitioner - Adult

Mountain Park Health Center is looking for Physician Assistants (PA) to join our well-established and accredited team of providers. As a large Federally Qualified Health Centers (FQHC) in Arizona, we are dedicated, to sustaining and improving the health of our patients by providing excellent, patient-centered primary care.

As a Physician Assistant (PA), you will provide diagnostic, therapeutic, and preventative health care services under the supervision of a physician.

Essential Functions:

• Provides direct patient care by obtaining medical history, performing appropriate physical examination, and creating an assessment and plan that may include ordering and interpreting labs, ordering imaging studies and developing treatment plan including referring to specialty care when needed.
• Administers or prescribes treatments and medications.
• Follows up and monitors patient’s progress throughout treatment plan.
• Documents the evaluation and plan of care.
• Provides patient education on a variety of health topics.
• Collaborates and consults with other in house allied health professionals to ensure coordinated patient care.
• Maintains regular and predictable attendance.
• Perform other duties as assigned.

Position Qualifications

Minimum Qualifications:

• Graduate of an accredited school for Physician Assistants
• Current Arizona PA License
• Current DEA Certificate
• Current CPR Certification
• Master of Medical Science in Physician Assistant Studies

Preferred Qualifications:

• Experience in primary care

We are looking for skilled Internal Medicine Physicians to join our well established and accredited team of providers located at our Baseline clinic site.

Internal Medicine Physicians will provide high-quality medical care to a wide variety of patients in a comprehensive, quality oriented, and supportive manner. Your role will encompass a broad range of outpatient primary care services, focusing on preventive care, health maintenance, and the management of acute and chronic medical conditions.
