The Wellness Plan Medical Centers (TWPMC) is currently looking to expand our services by recruiting for a BOARD CERTIFIED FAMILY MEDICINE PHYSICIAN. This position reports directly to the Chief Medical Officer. This role is responsible for health care management and provision of healthcare services to patients consistent with medical practice group principles.
Under the direction of the Dental Director, this position is responsible for performing a full range of dental hygienist's duties, protocols and procedures.
- This position provides preventive, therapeutic and intraoral services; including, but not limited to, performing hygiene duties such as examining teeth and other oral structures, cleaning teeth.
- Promotes dental health by completing dental prophylaxis, scaling, root planning as needed, providing oral cancer screening and radiographic studies.
- Charting conditions of decay and disease.
- Applying sealants, topical fluoride; and performs radiological procedures without the direct supervision of a Dentist.
- Assist in the training of Dental Assistants and other office staff; and development and promotion of opportunities and strategies to build the practice including development of the budget for the Hygienist practice.
- Assist with development and implementation of policies and procedures.
- Support a healthy and safe work environment for all employees.
- Maintain facilities and equipment cleanliness and safety and report and /or correct hazard, when necessary, in compliance with all state and federal regulations, including OSHA.
- Ensure compliance with policies and protocols of the organization as well as State, Federal and other regulatory.

The Licensed Master’s Level Social Worker (LMSW) position is responsible for creating optimal outcomes for the patient and family by managing complex psychosocial and economic co-morbidities. Through advanced practice skills, morbidities resources that reduce risks and serves as ambassador between medical centers and community. This position provides outreach activities, counseling, and engages community resources to assist non-compliant, chronic disease patients, complex patients and uninsured patients to comply with their providers prescribed health regiment. The LMSW provides crisis interventions and assists patients and families in understanding the implications and complexities of the patient’s medical condition and its impact on lifestyles. The LMSW position is a critical role and will also serve as a member of health center operations management team.
The Wellness Plan Medical Centers (TWPMC) is currently looking for a CLINICAL PSYCHOLOGIST (CP) who will be responsible for evaluating and diagnosing mental, emotional and substance use disorders in adolescents and adults. The CP will assist in shaping, formulating, directing and administering the Integrated Behavioral Health Services (IBHS) of the TWPMC clinics in collaboration with our Licensed Clinical Social Worker (LCSW) Coordinator along with the psychiatrist and other staff of the IBHS so that they work to accomplish a task and treatment plans for patients.
PRIMARY RESPONSIBILITIES:
- Evaluate and diagnose mental, emotional and substance use disorders in children, adolescents, adults and families.
- Utilize various techniques to identify disorders and determine treatments, including, but not limited to, interviewing the patient, observation of the patient and psychological testing and recognize symptoms and categorize each one in an effort to reach an eventual diagnosis.
- Gather and analyze information to select the best solution for each patient.
- Formulate and administers treatment programs specific to patient’s particular disorder.
- Assist to shape and direct the integrated behavioral health services of the clinics in collaboration with the LCSW/BHI Coordinator.
- Coordinate Behavioral Health Integration multi-disciplinary team, including the LCSW Coordinator, Psychiatrist and other staff so that they work to accomplish a task and treatment plans for patients.
- Detect changes in a patient’s circumstances or behavior.
- Identify the reason(s) or underlying principles driving a patient’s behavior and responses.
- Make observations and obtains information from relevant sources.
- Communicate with patients’ family members regarding ways they can participate and help with patients’ treatment plans.
- Provide pertinent information about patients to co-workers, supervisor and medical doctors.
- Develop specific objectives for each patient. Establishes long range goals for patients and plans to achieve the goals.
- Monitor information in electronic health record files.
- Keep up-to-date on new techniques and technology and applies them to evaluation and treatment processes.
- Provide instructions and guidance for subordinates.
- Ensure all processes are compliant with federal and state laws and regulations.
- Knowledge of biological, psychological and social development theories, diversity and cultural competency, interpersonal relationships, family group dynamics, mental disorders, additions, impacts.
- Travel to all TWPMC sites and throughout the organization’s service area and the State.
- Work flexible hours, including evenings and weekends as needed.
