Nurse Practitioner - Family Practice

This practice builds on previous knowledge, and skills and utilizes in-depth knowledge of physical assessment and management of client health care needs. This position works collaboratively with other health care professionals such as physicians and social workers.

PRINCIPLE DUTIES & RESPONSIBILITIES:

Working collaboratively with a physician, this position will carry out the major responsibility for the patient's health care needs including evaluation, management and follow-up.

Essential Functions Include:

• Obtaining complete medical history and physical data on patients
• Determine appropriate diagnostic procedures as needed
• Perform complete physical exams and record results in EMR, maintain and review charts
• Order appropriate lab and diagnostic procedures
• Educate patient and families regarding medications, health, and illness protection
• Perform other related work as desired.

PRIMARY CARE PHYSICIAN

JOB SUMMARY: The Primary Care Physician provides medical assessments and evaluations, provides treatment for emergent, urgent, and routine medical issues, provides, and supervises medical treatment for chronic medical conditions, and participates in organizational initiatives to help prevent diseases and injuries of patients in a community healthcare office setting. The Primary Care Physician works under the direction of the Medical Director and Site Administrator. Salary negotiable.

DUTIES & RESPONSIBILITIES:

• Examine patients regularly to assess their medical concerns and needs.
• Treat patients who have minor illnesses, acute and chronic health problems
• Collect, record, and maintain patient information, such as medical history, reports, or examination results.
• Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
• Examine patients or order, perform, and interpret diagnostic tests to obtain information on medical condition and determine diagnosis.
• Explain procedures and discuss test results or prescribed treatments with patients.
• Monitor patients' conditions and progress and reevaluate treatments as necessary.
• Direct and coordinate activities of nurses, students, assistants, specialists, therapists, and other medical staff.
• Refer patient to medical specialist or another practitioner when necessary.
• Other duties as assigned.
