Job Overview
The Nurse Practitioner, Care Management, provides care management and primary care medical services to high-risk patients. The NP manages the assigned patient panel.
Minimum Requirements
Nurse practitioner with an active, unrestricted CA license to practice medicine by the Nursing Board of California.
Minimum 2 years of experience and training as a nurse practitioner.
Minimum of 1 year experience in a high-risk specialty, Geriatric, Hospice, and/or Palliative Medicine, preferred.
Requires a valid and active license to practice medicine in the State of California.
Previous experience with computer-based systems required; Electronic Medical Records experience preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
Compensation
$138,257.60 - $172,806.40 annuallyCompensation Disclaimer

The range is subject to change. Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
