Family Medicine Nurse Practitioner

Hamdard Health Alliance is looking for a passionate and dedicated family nurse practitioner to join our primary care team and play a key role in providing care to the communities we serve. We provide primary care, psychiatry and counseling services for those most in need in Rogers Park, Skokie and Addison areas. Duties and Responsibilities:

• As a family medicine NP provides health care services on site and via telehealth to patients under the general direction of a licensed Physician if NP does not have full practice authority
• Provides quality care to those patients assigned to them that are consistent with evidence based guidelines and treatment protocols.
• Demonstrates clinical expertise through direct and indirect patient care.
• Investigates clinical problems and offers sound medical solutions.
• Contributes to the development, maintenance and change of the health care delivery system.
• Communicates effectively with other healthcare team members, patient, parent or significant others, based on their level of comprehension and understanding.
• Demonstrates the knowledge and skills necessary to provide age appropriate care by identifying medical, psychosocial and developmental needs and recommends appropriate interventions accordingly.
• Performs procedures utilizing medically approved approaches and equipment appropriate to the patient's age and size.
• Provides information and instructions at a level of understanding appropriate to the comprehension level of the patient and/or significant others.
• Actively participates in Quality Improvement activities
