About Community Health Net

Our clinic is equipped with advanced technology, including the Clarus500 Ultra-Widefield Retina Camera, CIRRUS OCT, and an auto-phoropter system, allowing our providers to deliver high-quality care and practice to the full scope of optometry.
Patient Population
Our practice serves patients across the lifespan—from pediatric to geriatric—with a strong focus on medical optometry. Many of our patients are adults managing chronic conditions such as diabetes and hypertension. Our goal is to keep patients healthy and engaged in their ongoing eye care.
About the Optometrist Role
As a member of our team, you will play a vital role in delivering comprehensive medical eye care, educating patients, and building long-lasting relationships. We are looking for a personable, community-minded optometrist who is passionate about patient-centered care and medical eye disease management.
Responsibilities
• Diagnose, treat, and monitor ocular conditions such as glaucoma, macular degeneration, cataracts, and diabetic retinopathy.
• Perform comprehensive eye exams with a strong emphasis on patient education.
• Utilize advanced office technology to deliver care at the full scope of your training.
• Refer patients to ophthalmologists when necessary and coordinate follow-up care.
• Manage or triage urgent cases during office hours and as needed after hours.
• Participate in staff meetings to set goals and review performance.
• Support community outreach initiatives and contribute creative ideas for patient engagement and practice growth.
Qualifications
• Doctor of Optometry (OD) degree.
• Licensed in the state of Pennsylvania (or eligible for licensure).
• New graduates welcome—mentorship provided.
• Passion for medical optometry and serving diverse patient populations.
• Motivated to contribute to practice growth and community engagement.
• Eligible for federal/state loan repayment programs through FQHC designation.

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
