We are looking for an Advance Practice Nurse (APN), Nurse Practitioner (FNP), or Physician Assistant (PA) to provide professional medical services to complement the delivery of integrated team-based, whole-person, trauma-informed primary health care services. This role will assess, maintain, and improve the health status of individuals in the communities served through timely access to evidence-based acute, preventive, and chronic disease care. Effectively collaborates with, and/or delegates to members of the interdisciplinary health care team, and provides indirect supervision of medical support staff.
