Contract Length: ongoing Shift Details: 5p-7a Schedule: 17-May 29-May 30-May 31-May Potential for more shifts Patients per day: 10-15 Patient population/age: Adults Department description: Trauma service –Dual boarded critical care trauma surgeons- Surgical trauma intensive care and general trauma patients EMR: Cerner Hospital bed size: 269 (Level 1 trauma) Skills required: Acute Care APP, ATLS required. Practice Setting: Hospital Requirements: Board Certification: Board Certified License required: AZ License DEA: Yes CSR: No Minimum experience required: 2 years Credentialing Timeframe: 30 days

Cerner Training: Mandatory, remote, approx. 4 hours, the week prior to start. Paid at provider's rate or $215/hr, whichever is less. In-person orientation may occur the day before or day of start.

1. Flights: Economy only. Airfare over $1,000 requires prior approval.

   • How to Get There By Air:

     • Flagstaff Pulliam Airport (FLG) is 10 minutes from the hospital, with direct flights to Phoenix and Dallas–Fort Worth.

     • Phoenix Sky Harbor Airport is approximately 2.5 hours away.

2. Lodging: Max $200/night for hotels or Airbnb/Vrbo. Direct bookings require canceled checks and proof.

3. Rental Car/Gas: Standard/economy rentals only, max $100/day. Gas to/from work and airport reimbursed. Mileage to facility is reimbursable; local mileage is not.

4. Parking: Hotel parking reimbursed. Airport economy parking or rideshare allowed; black car services not reimbursed.

5. Baggage Fees: Standard fees only; sports equipment not covered.
