Canyonlands Healthcare is a private, federally qualified, not for profit group with 7 clinics throughout Arizona. FQHC. Their corporate offices are located in Page, AZ.

POSITION:

Outpatient Clinic
Working within a patient centered care team
Seeing all ages of patients & whole spectrum of primary care
Working with acute and chronic health problems along with preventative care
Working with 4 other APP's
Will work autonomously occasionally
Clinic is open Monday - Friday 7:00am - 6:00pm
Possible option for 4/10's
No Call
No weekends
Mostly scheduled appointments
Some walk in patients
Spanish speaking provider would be ideal

COMPENSATION & BENEFITS:

Salary of $115,000 - $152,000 DOE

NHSC Loan Repayment: This position is at a National Health Service Corps (NHSC)-approved site. Base salary: $115,000 - $152,000/yr. Providers here may qualify for up to $75,000 in tax-free federal student-loan repayment over a 2-year full-time commitment — a total potential first-two-years package of up to $379,000 ($304,000 in base salary over two years + up to $75,000 in loan repayment). Award amounts depend on the annual application cycle and your eligibility.
