Position Summary

This provider will be a fully licensed and accredited Psychiatric Mental Health Nurse Practitioner (PMHNP) in their designated state. This role is responsible for diagnosing, triaging, and treating patients with behavioral health needs, including the prescribing and management of psychotropic medications. The PMHNP will monitor medication effectiveness for an ethnically diverse patient population with both medical and behavioral health conditions.

The provider will utilize brief intervention strategies, develop individualized treatment plans, and collaborate closely with medical providers, mental health staff, and other interdisciplinary partners to ensure coordinated and effective care.

Schedule and Location

The schedule is Monday through Friday, 9:00 AM – 5:00 PM. The position will be located in either our Evanston or Rock Springs clinic. The practitioner will provider services via telehealth to the other location.

Duties and Responsibilities

• Diagnose, triage, and manage behavioral health conditions
• Prescribe and manage psychotropic medications as clinically appropriate
• Provide medication management and ongoing monitoring of treatment response
• Deliver brief intervention strategies and supportive psychotherapy techniques
• Develop and maintain individualized treatment plans
• Collaborate with primary care providers, mental health staff, and external partners
• Participate in integrated, team-based care delivery
• Provide care to a culturally, socioeconomically, and ethnically diverse patient population
• Support MAT (Medication-Assisted Treatment) services or be willing to train and participate on a limited basis
• Maintain accurate, timely, and complete clinical documentation
• Ensure adherence to all clinical, ethical, and organizational standards

Qualifications

• Psychiatric Mental Health Nurse Practitioner (PMHNP) with an unrestricted license to practice in the state of Utah
• Three years of experience in mental health or behavioral health preferred
• Strong knowledge of behavioral medicine and psychiatric treatment modalities
• Ability to make timely and accurate clinical assessments and develop appropriate treatment plans
• Excellent verbal and written communication skills
• Strong organizational skills with the ability to multitask in a fast-paced environment
• Ability to work effectively as part of an interdisciplinary team, including medical, behavioral health, and administrative staff
• Experience working with culturally and socioeconomically diverse populations
• Demonstrated commitment to cultural competency and patient-centered care
• Flexible, self-directed, and aligned with the mission and values of Health West, Inc.
• Must pass background check with no evidence of unethical or immoral behavior
• Certification or willingness to be trained in MAT services preferred
