About the Role:
The role is responsible for reducing barriers to care by supporting patients in navigating health and social service systems. This position empowers patients with the tools, knowledge, and resources they need to advocate for themselves while serving as a critical link between patients, service providers, and community-based social services.

Additional differential of $10,000 annually for obtaining and maintaining Board Certifications in the areas of MDs and DO's - Infectious Disease, Endocrinology, Geriatrics, and Adolescence Medicine. This role(s) is a part of 1199 SEIU collective bargaining agreement

QualificationsFederally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
