Manages a team of clinical and non-clinical members who develop and maintain a program of comprehensive health care for patients of BCHC, to include preventive medicine, behavioral sciences, and primary care community health.
· Assists patients with chronic conditions in navigating and accessing all levels of chronic care services, including quality primary care, health education, and
behavioral health services
· Prepares and reviews medical histories and obtains data through interviews.
· Examines patients, formulates diagnostic plans, defines and orders required diagnostic testing. Interprets examination findings and test results and implements
treatment plans.
· Prescribes and assesses effectiveness of pharmaceuticals, other medications, and treatment regimens as appropriate for patient medical conditions.
· Participates in other care procedures according to training and demonstrated ability.
· Provides supportive and definitive care to patients with appropriate medical conditions in the scope of primary care, based on documented and demonstrated proficiency.
· Determines need for consultation and assists in medical care and treatment provided at the direction of other specialists.
· Determines needs for, and advises on kind and quantity of, medical supplies and equipment if applicable.
· Interviews patient, obtains patient history, updates all information and documents in EHR according to standards.
· Enters complete and accurate documentation for all assessment, diagnostic, treatment and follow up activities for each patient in the electronic medical record in accordance with BCHC policies and procedures.
· Establishes and maintains effective and ongoing communication with internal and external medical specialties, clinical support team and patients as a cornerstone of effective patient-centered, high quality primary care.
· Collaborates with care team members in efforts to complete pre-visit planning in advance of patient appointments to ensure care needs will be met, which may
include: Preventive care services needed; Educational needs; Updates to plan of care; Care Management needs; Care Coordination needs; and Community
referrals.
· Coordinates with Nursing Care Coordinator (NCC) and other care team members to identify highest risk patients and to maximize the management of patient
· Refers high risk patients to the physician for continued care and/or co-manages with the physician.
· Collaborates and consults appropriately with physicians, Chief medical Officer, and specialists as needed.
· Emphasizes health education and self management tools. Provides individualized patient education and health counseling regarding prevention, self management and health maintenance. Encourages patients and families to become educated consumers and to assume responsibility for their own health maintenance. Assures that patients/families are linked with community educational resources as needed. Provides (or provides for) group educational activities as appropriate.
· Provides clear, concise, comprehensive documentation.
· Provides leadership and oversight to clinical support team to ensure that strategies to collaborate with patients, providers, families and caregivers to enhance
continuity of care and effective utilization of health care resources are developed and maintained.
· Leads Morning Huddles on a daily basis
· Develops and ensures favorable relationships with all patients and their families.
· Communicates with patients in ways that culturally and linguistically appropriate using interpreters as needed.
· Participates in BCHC initiatives involving patient safety, quality, infection control, risk management and patient service, especially those related to provider issues.
· Participates in quarterly provider peer review activities as assigned by CMO.
· Participates in quality improvement projects and initiatives as requested by CMO and/or Chair, Board Quality Committee.
· Actively reviews evidence-based practice standards, health care advances and recommends such standards and advances for incorporation into BCHC medical practice.
· Identifies strategies to improve overall workflow and make day-to-day practice activities and patient flow more efficient.

• Directly affects improvements in patient well-being through accurate and timely diagnosis and delivery of evidence based, high quality
medical care. Ensure the delivery of competent, accurate, medical care and treatment to all patients as assigned.
• Ensures accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as
assigned.
• Ensures the delivery of all medical treatment and care is consistent with all regulatory, accreditation, and professional standards,
including clinical policies and quality initiatives.
• Understands and ensures all practices and procedures related to the delivery of medical services are consistent with established protocols
and procedures.
• Ensures and upholds the confidentiality and privacy requirements of all patient records, and manages all daily tasks and activities
consistent with HIPAA, state and federal laws and regulations, as well as within BCHC’s policies and procedures regarding confidentiality and security.
• Performs emergency life-saving procedures in cases such as cardiac arrest, respiratory arrest, massive hemorrhage, or other emergencies should they occur at BCHC.
• Supports BCHC care team delivery model.
• Assesses and manages patient’s pain per BCHC approved protocols and guidelines, ensuring that the management of pain and prescribing
of medication shall be individualized to the patient and follow Pennsylvania state guidelines Serves as collaborating physician for BCHC nurse practitioners
to support their role in primary care delivery.
• Supports BCHC’s commitment to education through mentoring residents, students and others engaged in learning initiatives at BCHC as
requested.
• Provides leadership and direction to non-physician center team members. Strives to meet or exceed all measurable standards of care,
including overall clinical, quality, and productivity goals.
• Develops and ensure ongoing, positive relationships and collaboration with other physicians, nurses, administrative and center operations staff. Ensures the success of each center operation through collaborative support and working relationships with all center operations staff.
• Develops and ensures favorable relationships with vendors, contractors and payer sources.
• Other duties as required.
