Adams County Health Center (ACHC) is seeking a full-time Nurse Practitioner or Physician’s Assistant. ACHC is a Federally Qualified Health Center located in Council, Idaho. We are looking for a goal driven team member who appreciates autonomy, variety, and flexibility. Our medical providers are supported by an entire team including behavioral health, dental, optometry, physical therapy, care management, laboratory, and pharmacy. ACHC takes great pride in improving the lives of people in our communities and was recently recognized nationally for increasing access to affordable health care!
Our patient population is all ages, pediatrics, adolescents, and adults. It keeps the day interesting.
RESPONSIBILITIES:
1. Assess patients and institute appropriate therapy within the guidelines established by the approved facility, state, and federal regulations and standards of care.
2. Performs therapeutic procedures such as administering injections, suturing, managing wounds and infections, etc. Patient assessment, diagnosis and therapeutic measures will be carried out in close association with the clinic physicians, appropriate specialists, and other care agencies in the community.
3. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
4. Collaborates with the Medical Director and/or other FNPs for patients who are beyond normal scope of practice/level of knowledge prior to instituting or prescribing therapeutic measures.
5. Provides immediate emergency treatment and/or refers as appropriate.
6. Assists with meeting quality measures by completing tasks related to ACHC initiatives.
7. Performs other duties as assigned.
Patient Centered Care:
1. Facilitate health and disease patient education.
2. Support patient self-management of disease and behavior modification interventions.
3. Coordinate continuity of patient care with external healthcare organizations and facilities, including referrals to specialty care providers, and continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
4. Promote clear communication among the care team and treat clinicians by ensuring awareness regarding patient care plans.

Nurse Practitioner/Physician Assistant shall be entitled to those employee fringe benefits currently offered to all ACHC employees including the following benefits which are subject to change at the discretion of ACHC:
Health Insurance
NP/PA will be provided with coverage under ACHC’s medical insurance plan offered to its employees under the same conditions as other ACHC employees.
Paid Time Off
NP/PA may take two hundred (200) hours of leave annually as paid time off ("PTO"). As provided herein, PA shall provide reasonable notice to ACHC of their intent to take PTO. PA shall utilize PTO in the year in which it is earned. Any unearned PTO will be forfeited at the end of the contract year. The above-listed PTO shall include time-off for sick time off, vacation and holidays.
Holiday
NP/PA will be off for the following paid holidays; 4th of July, Thanksgiving Day, Christmas Day or religious equivalent, New Year’s Day, Memorial Day and Labor Day,
Retirement Plan
ACHC employees will participate in the ACHC retirement plan. NP/PA will participate pursuant to the ACHC retirement plan documents and receive the plan-required employer match. All employees who work 20 hours per week must, pursuant to the plan document, participate in the ACHC retirement plan and make the minimum employee contribution required by the plan.
