This is an onsite part-time position 24 hours per week
The Nurse Practitioner will support the Transition of Care/ Medication Refill Team with refilling medications for patients without an assigned PCP to decrease hospital admissions and emergency room visits. Will also provide precepting by telehealth and scheduling in person/telehealth visits for those patients who need to have follow up care.
The Nurse Practitioner also will provide comprehensive, cost-effective health care to families which includes assessment, diagnosis, and treatment according to the standards of advanced nursing practice.
Educational Requirements:
Graduate of an ANA accredited Nurse Practitioner program. Master’s degree in nursing required.
Experience Required:
Minimum of 2 years of experience
Professional Licensure/Special Skills and Certification Required:
Nationally certified as a Family Nurse Practitioner
Massachusetts licensed
Current DEA/Mass Controlled Substance license for
prescriptive authority. BLS required.
Salary will be based on years of licensure
Reporting Relationship: Director of Population Health
