Must have chronic disease management in clinic experience and experience with providing primary care to the population of managing substances and mental health disorders *** All submissions must have a cover letter along with cv submitted** GROUP: We are in search of an experienced, dynamic Family Nurse Practitioner to join one of our primary care sites at La Clinica, a Federally Qualified Health Center, located in beautiful Southern Oregon. As a primary care medical home, we serve 28,000 patients annually throughout our six health care centers, 34-chair dental clinic, and mobile health center – a 40-foot converted RV. We are an innovative non-profit organization, with the focus on providing exceptional health care to all who have barriers to care, and include integrative behavioral and dental health care in our practices.
LOCATION: West Medford Health Center has a neighborhood health center feel, where a majority of our patients live in the immediate vicinity and walk to their medical home. Also providing comprehensive medical care to a large pediatric and adult population, this health center is located in one of our most impoverished Medford neighborhoods.

NHSC Loan Repayment: This position is at a National Health Service Corps (NHSC)-approved site. Base salary: $135,000 - $159,000/yr. Providers here may qualify for up to $75,000 in tax-free federal student-loan repayment over a 2-year full-time commitment — a total potential first-two-years package of up to $393,000 ($318,000 in base salary over two years + up to $75,000 in loan repayment). Award amounts depend on the annual application cycle and your eligibility.
