Greater Family Health is recruiting for three full-time family medicine Advanced Practice Providers (FNP or PA) for its Health Centers in Elgin, IL (373 Summits St., Elgin, IL. 60120), Streamwood, IL (135 E. Irving Park Rd., Streamwood, IL. 60107), and Des Plaines, IL (1515 Lee St. Des Plaines, IL. 60018.
This practitioner provides comprehensive primary health care services to a primarily low-income population, focusing on adult medicine with some pediatrics. Well care, sick care and chronic disease management are common. The position provides outpatient care full-time 4 days a week, with 2 evenings until 7:30 pm, and 1 Saturday a month from 8:00 am - 5:00 pm. The position has no hospital rounding responsibilities and is responsible for practice call no more often than 1 week of every 35 weeks.
As Greater Family Health (formerly Greater Elgin Family Care Center) is Joint Commission Accredited and PCMH certified, the practitioner is supported by a Care Team which includes a bilingual Medical Assistant, Clinical Operations Manager, Care Manager, Health Benefits Specialist, Clinic Coordinator and several additional professionals. NextGen is our EMR and we also use additional software to inform the practitioner in real time of any care gaps that the patient needs to have filled during each visit.
The salary range for a full-time Family Nurse Practitioner or Physician Assistant with Greater Family Health is $111,690-$130,000 annually and potentially higher based on education and experience.
