Position Title: Physician
Description: The Internal Medicine/Family Practitioner/Pediatrician is responsible for medical services provided at Celebrating Life Community Health Center.
Supervision: Reports directly to CMO.

Position Title: Certified Medical Assistant (Back Office)
Description: Perform administrative and certain clinical duties under the direction of physician. Prepare and maintain treatment rooms, drape and position patients, hand equipment, supplies, and instruments to physician. Takes vital signs, visions PAMS, assists with treatments, maintains equipment, visual fields and inventory supplies, including EMR, patient scheduling, charge slips and routine patient instruction and knowledge of CPR.
Supervision: Reports directly to the Back Office Supervisor
Responsibilities: The following statement for the position reflects the general duties considered necessary to describe the principal functions of the job as identified and shall not be considered a detailed description of all duties required that may be inherent in the position.
1. Schedules appointments.
2. Registers patient data in computer.
3. Sends appropriate information to various departments and medical offices.
4. Patient Relations.
5. Assures timeliness of services rendered to patients.
6. Clearly informs patient the name of physician who will render services.
7. Arranges for and assists patients in understanding instructions for all ancillary services.
8. Listens to complaints from patients regarding services rendered.
9. Demonstrates adequate triage technique in handling telephone calls from patients and inquiries regarding services rendered.
10. Appropriate management of filing records, recording telephone calls and prescription refill information in electronic medical records.
11. Checks floor stock and sample medications, discards expired medications appropriately, re-orders as needed.
12. Demonstrates accurate knowledge in management of emergency situations.

Job description
Under the direction of the Chief Behavioral Health Officer, provide the appropriate level of behavioral health and substance use disorder services for patients of Celebrating Life Community Health Center (CLCHC). Staff will provide services identified through a biopsychosocial assessment and promote health and wellness through services, advocacy and education with a focus on the needs of low-income patients. The position will also provide community behavioral health services to other clients seeking services at CLCHC. In addition, staff will be tasked with providing education and consultation to primary care and behavioral health providers on areas of addiction treatment, and seeking to enhance our integrated care model.

This clinician uses clinical skills to manage patients and supervise medical assistants in the performance of their duties. Based on Credentialing and Privileging manages pediatric, school-aged, adolescent and adult populations effectively in a rapidly growing private practice setting serving Medi-Cal patients. Works closely and effectively with front office and back office staff as well as other clinicians as needed to maintain effective patient management and efficient workflow. Must be enroll to CA Medi-Cal program or eligible.
