Job Description: Nurse Practitioner

- Assists in development of office policies and protocols. Follows and promotes compliance of others to policies and procedures in the performance of patient care
- Responsible for managing the treatment of chronically ill patients, provides family planning, well-child care and emergency medical care
- Diagnoses and treats common acute and chronic health problems
- Plans and prioritizes patient care delivery according to the patient's status and needs
- Provides accurate, thorough and comprehensive patient instruction relevant to the patient’s health maintenance plan
- Promotes quality of care through appropriate utilization of laboratory tests, x-ray procedures, EKGs and supplies
- Works directly with and seek consultations with precepting physicians
- Participates in concurrent review of patient records with precepting physician as regulated by the state of Tennessee
- Initiate referrals of patients to physicians, home health agencies and hospitals
- Abides by HIPPA regulations
- Assures the privacy and dignity of patients and their families
- Demonstrates appropriate health, safety and aseptic practices
- Is knowledgeable of Family Care emergency procedures and reacts accordingly
- Demonstrates initiative in identifying opportunities for self-development and enhancement of professional competency. Attend continuing education programs and activities and complete needed hours for certifying agency
- Participating in annual review of Family Care health care policies and procedures
Quality Patient Care & Clinical Leadership Responsibilities
The Nurse Practitioner (NP) is a clinical leader and essential provider within the healthcare team, responsible for delivering high-quality, patient-centered primary care. The NP works collaboratively with medical assistants, nursing staff, and administration to meet clinical quality measures, improve patient outcomes, and ensure strict compliance with Rural Health Clinic (RHC), Medicare, Medicaid, and other federal and state regulatory standards.
Responsibilities include, but are not limited to:
- Drive quality care metrics: Manage and oversee appropriate clinical documentation, preventive screenings, immunizations, diagnostic laboratory testing, comprehensive medication reconciliation, patient education, specialty referrals, and continuous follow-up care in accordance with evidence-based guidelines.
- Manage care gaps and outreach: Proactively identify care gaps across the patient panel. Design and execute clinical outreach and treatment plans for patients requiring annual wellness visits, chronic disease management, preventive services, vaccinations, and recommended screenings.
- Perform advanced clinical assessments: Evaluate patients by conducting comprehensive physical examinations, obtaining detailed medical histories, interpreting vital signs, and synthesizing complex clinical information to formulate diagnoses and treatment plans.
- Maintain electronic health records: Ensure accurate, comprehensive, and timely documentation within the Electronic Health Record (EHR) to support quality reporting, risk adjustment, and seamless continuity of care.
- Deliver patient-centered communication: Communicate complex medical information professionally, empathetically, and clearly to patients and families, promoting exceptional patient satisfaction and engagement.
- Lead quality improvement: Spearhead and participate in clinical quality improvement initiatives, peer reviews, staff meetings, and ongoing continuing medical education (CME) to optimize clinical outcomes and clinic performance.
Clinical Collaboration & Operational Leadership
The Nurse Practitioner is expected to contribute to the overall operational success and efficiency of the clinic by maintaining a collaborative, team-oriented leadership approach. When direct patient care schedules permit, the NP will engage in broader clinical oversight and operational support duties. These responsibilities may include, but are not limited to:
- Oversight of clinical environment: Monitor the clinical integrity, safety, and organization of exam rooms, laboratory spaces, and provider workstations.
- Clinical supply and protocol compliance: Ensure that medical equipment, clinical supplies, and emergency medications meet regulatory and operational readiness standards.
- Administrative and programmatic support: Assist with clinic development projects, regulatory audit preparations, inventory strategy, and policy development as requested by leadership.
- Interdisciplinary team support: Provide clinical mentorship, guidance, and backup support to medical assistants, nursing staff, and administrative coworkers to ensure efficient patient flow and exceptional customer service.
- Operational flexibility: Demonstrate a willingness to adapt and support operational needs outside the traditional clinical scope when necessary to ensure the clinic functions smoothly and meet community needs.
Communication & Chain of Command
Open, professional communication is vital to maintaining a productive and legally compliant clinical environment. Nurse Practitioners are expected to bring clinical queries, collaborate on complex cases, or address systemic patient care issues directly with the Medical Director or the Supervising Nurse Practitioner. For administrative matters—including scheduling, operational barriers, process improvement ideas, workplace issues, or staffing needs—the NP will communicate directly with the Office Manager.
Adhering to this established chain of command ensures effective problem-solving, maintains regulatory compliance, and fosters a collaborative workplace culture.
