Position Status: Full Time | Monday through Friday | 8:30AM-5:30PM

SPECEFIC DUTIES:
(MD/DO)
- Responsible for overseeing assigned patient’s care coordination including but not limited to specialty referral and follow up, case management, and enabling services. Provides professional medical services including examinations, diagnosis, and treatment planning for medical problems. Prepares and executes appropriate treatment plans and provides follow-up as needed while adhering to clinic policy. Consistently meets productivity expectations and procedures.
- Provides consultation when needed to nurse practitioners and physician assistants. Serves as supervising physician to no more than two mid-level providers. Proctors’ medical students as assigned.
- Maintains appropriate medical records documentation on all patients, in accordance with organizational policy. Consistently up to date on charting. Meets expectations of above 90% on weekly documentation report and maintains PAQ at less than 75 items at any given period.
- Actively contributes to care team by providing coverage to absent providers and being available to support staff and serving on improvement teams.
- Actively engaged in organizational initiatives such as 340b usage, Patient Centered Medical Home Certification, and Meaningful Use requirements.
- Works effectively in an integrated environment to coordinate care with dental and behavioral health professionals.

- Provides professional and quality medical services to the community and clinic patients. Consistently meets productivity expectations.
- Consults with supervising and/or on-call and consulting physicians regarding patients with abnormal or unusual medical conditions.
- Maintains appropriate medical records documentation on all patients in accordance with organizational policy. Consistently up to date on charting.
- Meets expectations of above 90% on weekly documentation report and maintains PAQ at less than 75 items at any given period.
- Actively contributes to care team by providing coverage to absent providers, being available to support staff, and serving on improvement teams.
