The County classification title is Health Services Manager II. This position is "At-Will" and exempt from Civil Service provisions.
Through collaboration amongst the partner agencies and the leadership of the Reentry Coordinator, Santa Barbara County will ensure individuals entering the criminal legal system receive multi-agency assessments and case planning to assist in their successful return to the community at the earliest stage possible based on their risk and needs, and the statutorily available options. This approach will assist in ensuring all diversionary options are maximized and individuals exiting the criminal legal system (pretrial and post sentence) have a robust discharge plan. This work will interface with work performed by multiple agencies between the Jail Discharge Planning Team, County Health Department, the Sheriff’s Office, Probation, County Executive Office, Public Defender’s Office, Managed Care Plans, California Department of Corrections and Rehabilitation, and a range of community-based service providers.
Under direction of the CalAIM Administrator, the Reentry Coordinator will lead the operational oversight and coordination of services for individuals reentering the community from the County’s two jails, with a primary focus on implementing CalAIM justice-involved initiatives, including Jail Reentry services. This role will manage the integration of service delivery between the County jails Enhanced Care Management (ECM) team and Community Supports, while working closely with the Medi-Cal managed care plan to ensure seamless transitions of care. They will provide clinical oversight to staff delivering reentry services including maintaining compliance with state licensure standards and clinical best practices. This includes supervision, training, and evaluation of clinical and care coordination staff, as needed, and will participate in regional and statewide planning efforts related to reentry services.
They will develop and implement protocols and workflows that support continuity of care for justice-involved individuals, ensuring these systems are responsive, equitable, and efficient. The position is also responsible for monitoring program data, ensuring accurate and timely reporting that aligns with CalAIM and Medi-Cal managed care plan requirements.
This position plays a critical leadership role in bridging behavioral health, public health, and correctional systems to support individuals during their transition from jail to community-based services, including coordinating care for Medi-Cal eligible clients. The ideal candidate will be a Licensed Behavioral Health Professional OR Nurse with strong program management and interagency coordination skills.

The current vacancies are for two (2) part-time (0.5 FTE) Pediatrician positions at the Santa Maria Health Center in Santa Maria and one (1) full-time Internal Medicine Physician at the Santa Barbara Health Care Center in Santa Barbara. The County classification title is Staff Physician.
As a licensed medical doctor, assumes independent responsibility for the clinical diagnosis and treatment of patients of the County of Santa Barbara Health (County Health) department with the all of attendant necessities of care, including but not limited to history, physical examination, ordering of tests, interpretation of testing, procedures, referral to specialists and overall case management of the patients’ medical conditions; performs related administrative duties to support County Health clinic and program operations; and performs other related duties as required.
DISTINGUISHING CHARACTERISTICS: Staff Physicians are licensed medical doctors who are Board Certified or eligible in Family Practice, Internal Medicine, Obstetrics/Gynecology, Pediatrics, or other recognized specialty consistent with County Health assignment. Professional services are provided in clinic settings and for a variety of public health programs. Incumbents report to the County Health Medical Director or designated Staff Physician, Supervising and receive administrative direction from a Health Center Administrator.

Health Navigators will work as a part of a collaborative inter-Departmental team to meet community health needs.
