The Position
Type of clinic/specialty: Home Based Primary Care/Assisted Living
The APC will provide assessment and management of acute and chronic health problems for residents of skilled nursing facilities, assisted living facilities, and for home-bound patients .
How many hours per week: 40 hours
Will you consider New Graduates: No, providers need - 6 months experience, ideally with a background as a CNA and RN in Assisted living, or nursing homes.
Whom else will the provider be working with?, is this a solo position: Will be solo, when traveling to patients homes. There is a MA that will assist with blood draws when at the assisted living.
What are the typical work hours: Clinic Hours: Monday - Friday 4x10s. Providers will split time between the assisted living and traveling to patients homes. Flexible schedule.

Salary range: $128,358 - $150,00 a year with Individual/Team Productivity Incentives, Individual/Team Patient Satisfaction Incentives and Retention bonuses Benefits:7% employer contribution pension plan (no match required)

NHSC Loan Repayment: This position is at a National Health Service Corps (NHSC)-approved site. Base salary: $128,358 - $150,000/yr. Providers here may qualify for up to $75,000 in tax-free federal student-loan repayment over a 2-year full-time commitment — a total potential first-two-years package of up to $375,000 ($300,000 in base salary over two years + up to $75,000 in loan repayment). Award amounts depend on the annual application cycle and your eligibility.
