Job Title: Family Practice Nurse Practitioner/Physician Assistant
Job Summary: The Family Practice Nurse Practitioner/Physician Assistant responsibilities include to diagnose and treat acute, episodic, or chronic illness, independently or as part of a healthcare team. May focus on health promotion and disease prevention. May order, perform, or interpret diagnostic tests such as lab work and x rays. May prescribe medication.
Job Duties:
This position is responsible for the following:
- Provide great care to SFMC’s patients of ages ranging from newborns to geriatrics.
- Collaborate and communicate effectively with Primary Care Providers and Healthcare personnel involved with patient care and treatment.
- Prescribe medication dosages, routes, and frequencies, based on such patient characteristics as age and gender.
- Diagnose or treat acute health care problems including, but not limited to, illnesses, infections, or injuries.
- Diagnose and treat chronic health care problems, such a high blood pressure and diabetes.
- Schedule follow-up appointments to monitor patients or evaluate health or illness care.
- Effectively monitor patients progress throughout treatment plan and care and provide continuous improvement on patient outcomes.
- Consult with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertise.
- Educate patients on importance of treatment plans and communicate side effects and risk of medications prescribed.
- Develop treatment plans based on standards of care and professional practice guidelines and implement to patient care.
- Demonstrate effective and detailed written documentation skills.
- Assist with development and improvement of programs as well as departmental policies, procedures and protocols.
- Order, perform, or interpret the results of diagnostic tests, such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays).
- Demonstrate excellent customer service with patients.
- Maintain current knowledge of state legal regulations for nurse practitioner practice.
- Effectively write clinical reports regarding patient diagnoses and prognoses.
- Able to multitask and manage assigned projects and tasks as directed.
- All other duties and responsibilities as assigned.
JOB REQUIREMENTS:
- Must possess and maintain a valid State of California Registered Nurse License.
- Must possess and maintain a valid State of California Nurse Practitioner Furnishing License.
- Ability to work under pressure.
- Ability and willingness to treat all patients and co-workers with the utmost kindness and consideration in the most trying situations.
- Ability to handle multi-functions.
- Promotes and believes in SFMC.
- Ability to relate to public regardless of ethnic, religious and economic status.
- Must possess or obtain a Valid California Driver’s License.
- Must be able to perform all essential duties of the job with or without reasonable accommodation.
QUALIFICATIONS, EDUCATION, AND EXPERIENCE:
- Must have a Master’s Degree in Nursing from an accredited school.
- Ability to demonstrate thorough knowledge and expertise in reporting laws as well as all legalities of care relevant to the specific population serviced.
- Ability to demonstrate excellent independent judgment, problem-solving, analytical and collaborative skills in order to organize and prioritize multiple tasks, providing patient centered leadership and direction.
- Must have the emotional intelligence that may include, but not limited to, patience, sensitivity and the compassion to work with patients that may be stressed, anxious, and difficult.
- Ability to work as a team.
- Ability to demonstrate knowledge and willingness to engage in learning about various spiritual, ethnic, and culturally diverse backgrounds.
- Bilingual in English/Spanish is preferred.
- Must have strong computer skills.
- Must demonstrate detailed, effective writing/documentation and verbal skills.

SALARY: $140,000 - $180,000 per year, DOE
