Medical Director
Healthy Community Health Centers (HCHC) has been operating since 2008 as a Federally Qualified Health Center serving the City of Harrisonburg, Rockingham County, and the surrounding area.
HCHC serves approximately 15 thousand community members by providing adult and pediatric primary care, acute care, chronic disease management, wellness / preventative services, women’s health services, including pre and post-natal care, HIV/AICS support services, behavioral health, community resource coordination, translation services, outreach and enrollment support services, medication and transportation assistance, including access to medication cost assistance, on site and collaborative pharmacy services with community partners and more.
HCHC has an open position for a skilled physician leader to represent the organization as Medical Director. This position will provide direct supervision and support to the HCHC medical provider team as well as provide access to care for patients in our community. The administrative role encompasses 50% of the work time for this position.

The Dental Hygienist is responsible for the removal of hard and soft deposits of stains, calculus and plaque from teeth as well as administering preventive dental care in a community health care setting.
The Dental Hygienist is board certified or board eligible licensed practitioner who is responsible for providing patient care and working with health center providers and staff to ensure quality outcomes for our patients.
The Dental Hygienist will work with the Dental Director and other organizational leaders to maintain center policies, procedures and protocols.
This position is eligible for the National Health Service Corps (NHSC) Loan Repayment Program (LRP). Dental Hygienists have the opportunity to have their student loans repaid, while earning a competitive salary, in exchange for providing healthcare in communities with limited access to care.

The role of Nurse Practitioner at the HCHC is to provide high quality primary health care patient services to support the Mission and Vision of HCHC. The Nurse Practitioner will support HCHC’s organizational values and strategic goals including care delivery thru our patient centered medical home model. The Nurse Practitioner will document in the electronic health record per HCHC protocol. The Nurse Practitioner will provide services in compliance with applicable regulatory and licensing agencies.
Responsibilities
· Monitor and provide counsel, collaboration and consultation to other clinical and administrative staff in order to achieve the best possible clinical outcomes for Health Center patients.
· Support the HCHC Quality Improvement plan and goals including initiatives to support patient safety, the patient experience, access to care, and initiatives to improve the health of the community.
· Participate in HCHC’s Peer Review process.
· Provides comprehensive primary care within the collaborative agreement documents scope of practice defined by HCHC privileging protocol for the medical specialty. Care will be delivered in full accordance with HCHC policies and protocols, including:
§ Provide wellness, chronic disease, same day, and emergency services in accordance with the patient’s needs.
§ Provide a clear assessment and safe treatment plan for each patient encounter that takes into account the patient’s circumstances.
§ Order, perform, and interpret routine laboratory and diagnostic imaging tests and notify patients of results per HCHC protocol.
§ Provide access to primary care thru Telehealth and phone.
§ Support wellness and restoration through self-care, continuity of care and care coordination.
§ Refer patients for specialized care based upon an identified need.
§ Incorporate recommendations from consultants into treatment plans.
§ Complete documentation in the EMR per HCHC protocols.
§ Comply with EMR maintenance and upgrade activities.
· Collaborates with physicians and clinical team in accordance with collaborative agreement documents to plan and coordinate patient care.
· Provides professional expertise for in-service training and Quality Improvement projects.
· Actively participates in Quality Improvement initiatives and Peer Review
· Obtains and maintains appropriate privileges at area community hospitals.
· Demonstrates commitment to HCHC’s recognition as a Patient Centered Medical Home and its associated excellence standards by modelling excellence in all clinical, administrative and team process relationships, addressing service excellence deficits.
· Provides call coverage, cross coverage, Saturday clinic coverage and other shared work.
· Other duties and responsibilities as assigned.

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
