Community Health Connections (CHC), a multi-site, non-profit Federally Qualified Health Center (FQHC) with decades of experience, is seeking a Family Nurse Practitioner (FNP) to join their team. This role involves coordinating comprehensive healthcare for a specific patient population and providing direct patient care under the general direction of a supervising physician and Medical Director. The FNP will contribute to CHC's mission of delivering compassionate, quality healthcare to 36 communities in North Central Massachusetts, regardless of income or insurance status.

Requirements
Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment. Half-time service is also eligible at a reduced award (up to $37,500). Eligibility extends to NPs, PAs, CNMs, and physicians with qualifying education debt.
