Community Health Partnership of Illinois is a is a non-profit organization that provides primary health care to migrant farmworkers and other medically underserved populations throughout northern and central Illinois that specializes in primary preventative medical, dental, and behavioral health service. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.
At CHP the Family Nurse Practitioner (FNP) is responsible for providing primary care to patients of all ages, from infants to elderly individuals. FNPs work in a variety of healthcare settings, including assigned clinic and local outreach events.
The duties of a Family Nurse Practitioner may include conducting physical examinations, ordering and interpreting diagnostic tests, diagnosing and treating common illnesses and injuries, prescribing medications, managing chronic conditions, providing preventive care and health education, and referring patients to specialists when necessary. FNPs also collaborate with other healthcare providers to ensure comprehensive and coordinated care for their patients.
In addition to clinical responsibilities, Family Nurse Practitioners may also be involved in conducting research, participating in quality improvement initiatives, and contributing to the development of healthcare policies and practices. FNPs must possess strong communication and interpersonal skills, as they work closely with patients and their families to provide compassionate and effective healthcare.
Qualified candidates for this position must hold a master's or doctoral degree in nursing, be licensed as a Registered Nurse, and have certification as a Family Nurse Practitioner. Prior experience working in primary care or a related field is typically required. FNPs must also stay current with advances in healthcare and maintain their licensure and certification through continuing education and professional development activities.
Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
