Volume: 2 visits per hour, with bonus opportunity for exceeding

About the Role

The Nurse Practitioner or Physician Assistant will play a crucial role in delivering primary care services, examining patients, and collaborating with healthcare providers to ensure optimal patient outcomes.

Responsibilities

• Assess and examine patients to determine health status and diagnose conditions
• Document patient care and communicate effectively in the EMR
• Order, perform, and interpret diagnostic tests and screenings
• Develop and implement treatment plans, including prescribing medications and therapies
• Perform procedures such as injections, immunizations, suturing, and wound care
• Educate patients on diagnoses, treatments, preventive care, and healthy lifestyle choices
• Monitor patient progress and modify treatment plans as needed
• Refer patients to specialists or community resources when appropriate
• Collaborate with supervising physicians per standardized procedures
• Maintain compliance with scope of practice, HIPAA, and regulatory requirements
• Participate in quality improvement, peer review, and organizational initiatives
• Supervise and support clinical staff, students, and interns as needed
• Maintain licensure, continuing education, and professional competency
• Provide emergency care and perform related duties as required
• Attend required meetings, trainings, and occasional travel as needed

Qualifications

• Active unrestricted CA FNP or PA License, DEA and BLS
• Must have excellent interpersonal skills including the ability to interact with staff and providers at other medical facilities

Preferred Skills

• 2 years previous experience working as a Nurse Practitioner preferred

About the Role

The Physician will be responsible for delivering comprehensive medical care, including diagnosis, treatment, and patient management, while ensuring adherence to medical standards and practices.

• Evaluate, diagnose, and treat patients’ acute and chronic medical conditions

About the Role

The OB/GYN Physician provides comprehensive outpatient gynecologic and limited prenatal care within a Federally Qualified Health Center (FQHC) setting, with no labor and delivery responsibilities. This position delivers high-quality, patient-centered care to a diverse, medically underserved population, regardless of ability to pay, consistent with the FQHC mission and federal Section 330 requirements.

The physician works within a team-based, integrated care model to address medical, behavioral, and social determinants of health, improving health outcomes and advancing health equity.

• Position is eligible for Federal Tort Claims Act (FTCA) malpractice coverage.
• Eligible for National Health Service Corps (NHSC) loan repayment programs, if Physician is applicable.
• Must support and adhere to the mission, vision, and values of the FQHC.

Essential Functions

The following duties are fundamental to the position (Duties may include, but are not limited to, the following):

Direct Patient Care

• Provides comprehensive outpatient gynecologic care, including preventive health services, screenings, and treatment of acute and chronic conditions
• Performs well-woman exams, Pap smears, breast exams, and contraceptive counseling
• Diagnoses and manages gynecologic conditions such as abnormal bleeding, infections, menopause, and reproductive health concerns
• Performs in-office procedures including colposcopy, biopsies, and contraceptive device placement/removal
• Provides pre- and post-natal care in an outpatient setting, excluding labor and delivery
• Orders, interprets, and evaluates laboratory and diagnostic tests
• Refers patients to specialty or hospital services and ensures continuity of care

Care Coordination and Team-Based Care

• Collaborates with interdisciplinary care teams including primary care, behavioral health, dental, and case management staff
• Participates in team huddles and coordinated care planning
• Facilitates referrals and follow-up care within and outside the organization

Social Determinants of Health and Patient Support

• Identifies and addresses social determinants of health, including barriers related to housing, food, and transportation
• Provides culturally competent and trauma-informed care
• Utilizes interpreter services and appropriate communication tools to ensure equitable access
• Educates patients on preventive care, reproductive health, and treatment plans

Documentation and Compliance

• Complies with clinical guidelines, clinical and all company policies, and regulatory standards
• Documents patient encounters accurately and timely in the electronic health record
• Supports appropriate coding, billing, and documentation practices

Quality Improvement and Productivity

• Meets established productivity standards for patient care
• Supports organizational quality goals, including Uniform
• Data System (UDS) measures
• Participates in quality improvement, peer review, and patient safety activities

Community and Organizational Engagement

• Supports outreach and community health initiatives

• Adheres to Health Resources and Services Administration (HRSA) and FQHC program requirements

• Follows all applicable federal, state, and local healthcare laws and regulations

Supervision

• Received: General administrative and medical direction from a Medical Director or Chief Medical Officer
• Exercised: May provide clinical direction to nursing staff, medical assistants, residents, or students; May be assigned peer review activities or Physician Supervision as needed

Minimum Qualifications

Education:

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
• Completion of an accredited Obstetrics and Gynecology residency program

Licensure & Certification

• Current, unrestricted license to practice medicine in the State of California
• Board certified or board eligible in Obstetrics and Gynecology (ABOG or AOBG)
• Current DEA registration
• Basic Life Support (BLS) certification (ACLS may be required based on clinic policy)

Experience:

• Completion of OB residency program
• Familiarity with team-based care models and population health approaches preferred

Preferred

• 2+ years performing in-office gynecologic procedures
• Experience in an outpatient OB/GYN or gynecology-focused practice preferred
• Experience working in an FQHC, community health center, or public health setting highly desirable
• Demonstrated experience working with underserved, Medicaid/Medi-Cal, or vulnerable populations preferred
• Experience with NextGen EHR

Knowledge of:

• Strong clinical knowledge of gynecologic care and outpatient reproductive health management
• Knowledge of preventive care guidelines and women’s health screening standards
• Understanding of social determinants of health and their impact on patient outcomes
• Familiarity with HRSA/FQHC program requirements and quality reporting (UDS) preferred

Ability to:

• Ability to interpret diagnostic results and develop evidence-based treatment plans

·Apply a comprehensive knowledge of primary care or specialty care to treatment of illnesses.

·Orders appropriate diagnostic, screening, and follow-up studies and testing as indicated by the patient’s illness.

·Utilize knowledge of pharmacology when prescribing medications.

·Make referrals to community based resources, as needed.

·Utilize physician consultation or referral for any patient, task, procedure, or diagnostic problem that the mid-level provider determines exceeds his/her level of competence.
