Position Summary

The provider will be a fully accredited Advanced Practice Provider (APP), licensed to practice in their respective state, with board certification in Family Practice preferred. The provider is responsible for delivering direct patient care within designated service areas of Health West, Inc.

This role works collaboratively with the Director of Advanced Practice Providers and the Chief Medical Officer, who provide guidance, support, and performance evaluation. In the course of required duties, exposure to bloodborne pathogens may occur due to direct patient care, trauma response, and laboratory procedures.

Schedule

The schedule is Monday through Friday, 9:00 AM – 5:00 PM, with the option to begin at 32 hours per week and transition to 40 hours per week as the provider’s patient panel grows.

Detailed Responsibilities

• Provide direct primary medical care, with an emphasis on women’s health
• Counsel and advise patients and their families on treatment, care, and lifestyle adjustments
• Refer patients to appropriate ancillary and supportive services
• Participate as a member of the Quality Assurance (QA) committee when assigned
• Maintain strict confidentiality of all information obtained while performing professional duties
• Supervise, mentor, train, and evaluate mid-level providers as assigned
• Perform additional duties or assignments as outlined in individual employment agreements
