Full-Time Nurse Practitioner

The Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.
Responsibilities:
- Involve with direct patient physical assessment, education, and health planning
- Maintain continuity of care by regular patient follow-up
- Assist in the coordination of services with other disciplines and patients' insurance plans
- Advocate for patient in arrangement of medically necessary services
- Communicate with community resources to ascertain adequate assistance for patients in need
- Refer patients to specialty services when necessary
- Consult with collaborating physician and other members of the health care team to establish plan of care
- Participate in quality assurance process to assure optimal care delivery
- Provide comprehensive case management services that include emphasis on health promotion and disease prevention
- Monitor patient outcome in accordance to the practice protocols
- Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
- Act as a resource person for the nursing staff
- Conduct education in service on practice issues
- Perform other duties as assigned
Minimum Qualifications:
- Graduated from approved Nurse Practitioner Program
- Licensed as Nurse Practitioner in New York State
- Basic CPR (BCLS) certification required
- At least 3 years of clinical experience as a primary care Nurse Practitioner
- Board/Specialty Certification preferred
- Bilingual in English and Mandarin Chinese required
Compensation and Benefits:

Full-Time Licensed Clinical Social Worker (Bilingual in English/Chinese speaking)
Reports To: Chief of Mental Health
Responsibilities:
- Conduct psychosocial assessment, may include diagnostic interview
- Develop and implement treatment plan
- Provide psychotherapeutic interventions, including individual psychotherapy, group psychotherapy, play therapy, family and couple counseling, and other evidence-based treatments
- Provide crisis intervention
- Provide behavioral health consultation
- Provide case management services
- Provide agency referral and follow up
- Provide community outreach/education services
- Maintain treatment records in accordance with Health Center’s guidelines
- Participate in agency Quality Assurance activities and case conference
- Supervise LMSWs, interns, and volunteers
- Attend departmental meetings
- Ability to work on weekends and during late hours
- Perform other duties as assigned
Minimum Qualifications:
- Master’s Degree in Social Work from an accredited social work school
- New York State Licensed Clinical Social Worker (LCSW) License required
- At least 2 years of experience as a social worker in mental health field
- Basic CPR (BCLS) Certification required (Health Center will provide if needed)
- Effective verbal and written communication skills a must
- Bilingual in English/Chinese (Mandarin and/or Cantonese) required
