Greater Family Health is recruiting for a certified pediatric nurse practitioner (CPNP) to provide care at our 300 McHenry Rd., Wheeling, IL, 60090 health center.
This practitioner provides comprehensive primary health care services to a primarily low-income population, focusing on pediatrics. Well care, sick care and chronic disease management are common. The position provides outpatient care full-time 4 days a week, with 2 evenings until 7:30 pm, and 1 Saturday a month from 8:00 am - 5:00 pm. The position has no hospital rounding responsibilities and is responsible for practice call no more often than 1 week of every 35 weeks.
As Greater Family Health is Joint Commission Accredited and PCMH certified, the practitioner is supported by a Care Team which includes a bilingual Medical Assistant, Clinical Operations Manager, Care Manager, Health Benefits Specialist, Clinic Coordinator and several additional professionals. NextGen is our EMR and we also use additional software to inform the practitioner in real time of any care gaps that the patient needs to have filled during each visit.
The salary range for a full-time Family Nurse Practitioner or Physician Assistant with Greater Family Health is $111,690-$135,000 annually and potentially higher based on education and experience.
