Position Summary

Tampa Family Health Centers is seeking a Nurse Practitioner (APRN) to join our multidisciplinary care team. This role is responsible for managing common acute and chronic health conditions through comprehensive assessment, diagnosis, treatment, and follow-up care.

The APRN functions as an interdependent member of the healthcare team, collaborating with physicians, advanced practice providers, nursing staff, and care coordinators. This position practices to the full scope of licensure in accordance with State and Federal regulations, supporting TFHC’s Patient-Centered Medical Home (PCMH) model.

Essential Responsibilities

• Provide comprehensive patient care through health promotion, disease prevention, and health restoration
• Conduct patient histories, physical examinations, and medical assessments
• Diagnose acute and chronic conditions and develop individualized treatment plans
• Order, interpret, and review diagnostic studies, including laboratory and radiographic tests
• Prescribe medications and manage follow-up care in accordance with clinical guidelines and licensure
• Provide education and counseling to patients and families regarding health maintenance and illness prevention
• Collaborate with physicians and interdisciplinary care team members to coordinate patient care
• Participate in daily team huddles and ongoing care planning
• Serve as a preceptor and mentor to Physician Assistants, nursing staff, and APRN students, as applicable
• Float between clinical locations as operational needs require
• Participate in quality improvement initiatives to address care gaps and enhance patient outcomes
• Uphold patient rights, privacy, dignity, and safety at all times
• Promote teamwork, professionalism, and positive patient and community relations
• Maintain compliance with HIPAA, State, Federal, HRSA, and TFHC policies and regulations
