Covenant House's Federally Qualified Article 28 Health Clinic is a unique healthcare program that provides comprehensive physical health, social work, nutrition, mental health and substance use services to homeless young adults.
As a member of Covenant House New York's Federally Qualified Health Clinic team, the part-time Psychiatric Nurse Practitioner will be responsible for psychiatric evaluations, medication management and brief psychotherapy for youth residing in Covenant House's Residential Programs. He/she will work closely with the Medical Providers providing clinical, anticipatory guidance and health-related education to young adults and will help coordinate care for patients living with medical complexity, disabilities or serious emotional or psychiatric disturbances. The patient population will consist entirely of young adults, ages 16-25 years old; thus, familiarity and a desire to work with this population is essential. This position will report to the Medical Director and will work collaboratively with the Director of the Mental Health Department. Part time hours are flexible with the days of the week. Ideally one day in person and remaining hours virtual but there is flexibility.
Reporting to the Director of Mental Health, the Health Center Social Worker, who is CASAC-certified, is responsible for providing psychosocial services, including crisis intervention, risk/psychosocial assessments, connecting youth to resources in the community, and short-term supportive counseling to a diverse youth homeless population and community patients. The Health Center Social Worker will also lead the continued integration of mental health, primary care, and substance use services for clients utilizing our Federally Qualified Health Center and residing in our crisis shelter. The Health Center Social Worker should have a focus and understanding of trauma-informed care and working with clients who have experienced significant trauma. The Health Center Social Worker will participate in interdisciplinary case conferences, work with primary care providers, and assist with program development to ensure high-quality delivery of services.
The Health Center Social Worker provides psychosocial services using a variety of interventions, including evidence-based models such as TF-CBT, DBT, and Motivational Interviewing, to young adults aged 16-21 residing in a 120-bed RHY shelter and aftercare services, as well as community clients utilizing our Federally Qualified Health Center.
As this is a new position, the Health Center Social Worker must be comfortable in a rapidly changing environment where processes and protocols are being developed and refined as the program operates. Flexibility, excellent communication, and problem-solving abilities are critical to successfully fulfilling the requirements of this position. This position includes opportunities to be trained and certified in evidence-based practices (EBPs) and continuing education credit opportunities through the InUnity Behavioral Health Coalition, of which CHNY is a member. As a Covenant House New York's Health Center team member, the full-time Medical Assistant (MA) works closely with the Clinical Providers, providing clinical, anticipatory guidance to children and families. The MA provides these services during visits to our FQHC Article 28 health center in Midtown Manhattan. Care is delivered in collaboration with Covenant House New York's outstanding teams of social workers, education specialists, physicians, nurse practitioners, and other staff.
The Medical Assistant is competent in performing the following functions appropriate to the age of the patients: maintaining a safe and clean care environment, improving quality, and assisting in training the new students and volunteers. The assistant actively and consistently contributes to department operations and communications, behaves consistently with Covenant House's mission and objectives, and performs other duties as requested.
What you'll do
(Responsibilities to include but not limited to the following)
- Interview patients to determine the reason/purpose of the visit and to obtain any pertinent medical information related to the visit
- Prepares patients for examination by performing preliminary physical tests, taking blood pressure, weight, and temperature.
- Record the information provided by the patients, along with vital statistics and information such as test results, in their electronic health records.
- Escorts patients to the examination rooms and prepares them for the physician.
- Order medical supplies and equipment, prepare purchase requisitions, and maintain working relationships with Covenant House's medical supply houses and purchasing departments.
- Responsible for the organization, stocking, and preparation of examination rooms.
- Maintains and orders all health-related forms
- Performs venipunctures as needed and labels specimens.
- Delivers medications to residential floors as needed
- Collect blood or other laboratory specimens, log them, and prepare them for testing.
- Explain treatment procedures, medications, diets, and physicians' instructions to patients.
- Prepare treatment rooms for patient examinations, keeping the rooms neat and clean and stocked with required medical supplies.
- Clean and sterilize examination rooms and instruments and dispose of contaminated supplies.
- Schedule appointments for patients when covering the front desk.
- Change dressings on wounds.
- Greet and record patients arriving at the center.
- Perform general office duties such as answering telephones, dictating, and completing insurance forms.
- Assist with inventory and order medical, lab, and office supplies and equipment.
- Maintains and is responsible for all storage areas, including orderliness and rotation of storage closet and storage room.
- Perform routine laboratory tests and sample analyses.
- Set up medical laboratory equipment.
- Operate electrocardiogram (EKG) and other equipment to administer routine diagnostic tests.

The LPN will coordinate with the health care team to facilitate the highest quality of health services for patients. LPN will also provide telephone triage, arrange lab work, administer medications, and provide educational outreach. This position will be hired full-time within the Health & Wellness Center and will work with patients during all operating hours, including weekend hours. This position requires in-person presence at CHNY’s Manhattan location; there is no opportunity for remote work.

The Medical Receptionist is responsible for direct patient contact, including initial greeting, patient appointment scheduling, registration, insurance verification, communicating data to provider & staff, sending referrals, re-calling patients for follow-up appointments. Competent in performing the following functions appropriate to the age of the patient, maintains a safe and clean care environment, quality improvement, assists in the training of the new students and volunteers. Actively and consistently contributes to department operations and communications, behaves in a manner consistent with the mission and objectives of Covenant House and performs other duties as requested.

The Director of Mental Health Services will lead CHNY’s mental health services department which includes on-site and virtual therapeutic and crisis responses services across all of CHNY’s programs, a robust anti-human services team, mental health day treatment programming and supportive housing evaluation and case management, as well as integrated behavioral health care. This role leads CHNY’s efforts to remain on the cutting edge of providing evidence based and best practice driven services and supports by ensuring that models are best in class and meet the changing needs of our youth population. Additionally, the Director plays a key role in providing support across all of CHNY’s program areas so that we are meeting youth where they are and providing trauma informed practices throughout.
Flexibility in schedule, with most days on site at our 41st Street location, ability to travel to the Bronx as needed, on-call by email and phone in off-hours, and excellent communication and problem-solving abilities are critical to successfully fulfilling the requirements of this position.
Administrative
- Creates and manages mental health and anti-human trafficking services department that embodies CHNY’s mission and values as well as effectuates healing and positive change in the lives of young people by providing direct supervision, clinical supervision, leading evidence based model implementation efforts and ensuring ongoing fidelity, ensuring best practices are followed, leading and supporting ongoing Covenant House program model efforts, and leading specific efforts on CHNY’s Strategic Roadmap
- Spearheads and supports ongoing collaborative efforts between CHNY program departments, including facilitating collaborative discussions across departments and teams as well as managing the department’s on-call schedule so that 24/7 programs are supported when there are not social workers on site.
- Develop and conduct trainings and workshops for staff and external stakeholders
- Represent CHNY on panels, with potential funders, and with other stakeholders. Including, attending coalition and task force meetings.
- Oversees data management systems and ensures that all documentation is completed in ETO and in eCW (Electronic Health Records) in a timely manner. Including developing and providing ongoing management and support of the departmental responsibilities for timely and accurate documentation for billing.
- Responsible for managing the department’s budget and supporting fundraising efforts, along with ongoing grant management support, such as narrative writing, grant activities reports, and outcome setting and obtainment.
