Nurse Practitioner - Psychiatric Mental Health

PRIMARY PURPOSE

The Psychiatric Nurse Practitioner (PMH-NP) provides compassionate, supportive, patient focused psychiatric treatment to adolescents and adults in an outpatient setting per an integrated model of care for patients of Lorain County Health & Dentistry (LCH&D). The PHM-NP is expected to work in collaboration with LCH&D primary care providers (including those who provide Medication Assisted Treatment), behavioral health providers, and other LCH&D staff. The PMH-NP will refer to external providers and treatment facilities, as appropriate, when patient needs exceed LCH&D services. At LCH&D, in the spirit of teamwork, the PMH-NP is expected to respond timely to consultative requests, or other requests for input and expertise, from primary care or behavioral health providers. From time to time, the PHM-NP may be requested to conduct administrative chart reviews, during work time, of psychotropic prescribing practices among primary care providers for development purposes.

ESSENTIAL FUNCTIONS

• Review patient’s medical history.
• Elicit relevant information from patient regarding current and prior psychiatric conditions.
• Elicit relevant information from patient regarding their lifestyle and behavior.
• Elicit relevant information regarding family medical and mental health history.
• Perform psychiatric evaluation and routine checkups and screenings.
• Prescribe, direct, and/or administer psychotherapeutic treatments or medications to treat mental, emotional, behavioral, or substance use disorders.
• Monitor medication management of patients evaluated and referred by the Behavioral Health Team.
• Provide appropriate psychiatric treatment to adolescents and adults.
• Collaborate with the legal guardian of minors
• Monitor and teach patients the use of medications according to best practices.
• Refer patients to specialty care if needed for complex medical conditions.
• Provides psychiatric information and collaborates with primary care providers.
• Collaborate appropriately with providers and behavioral health team.
• Participate in Peer Review.

PRIMARY PURPOSE

The MAT Care Coordinator at Lorain County Health & Dentistry (LCH&D) plays a central role in supporting patients engaged in Medication-Assisted Treatment (MAT) services. This position is responsible for meeting with patients during MAT orientation and providing ongoing support in coordination with Primary Care Providers (PCPs) during follow-up visits. The MAT Care Coordinator delivers trauma-informed, culturally responsive care coordination to promote patient engagement, adherence to treatment, and positive health outcomes.

This role also supports the growth and development of the MAT program through community outreach, marketing efforts, and partnership-building. This position is grant-funded and focused on increasing access to care, improving retention, and expanding awareness of MAT services within the community.

Essential Functions:

Patient Care Coordination

• Conduct MAT program orientations, reviewing program expectations, treatment structure, and available supports
• Meet with patients during PCP visits to reinforce treatment adherence and address barriers to care
• Collaborate with behavioral health providers, PCPs, and additional care teams to support integrated, patient-centered care
• Provide trauma-informed, culturally sensitive support to patients with substance use disorders
• Assist with coordination of internal and external referrals, including behavioral health and community resources
• Conduct outreach to patients who miss appointments to support re-engagement in care

Medication Coordination and Prior Authorization

• Coordinate with onsite and external pharmacies to support timely access to MAT medications
• Schedule and track deliveries of injectable medications to ensure availability for patient appointments
• Verify that prescriptions are sent accurately and timely, including correct medication, dosage, and pharmacy location
• Complete prior authorizations for MAT medications and follow up to ensure timely approval
• Assist patients with enrollment in copay assistance programs and other medication access resources as needed
• Collaborate with nursing staff to track medication deliveries and confirm medications arrive at the correct location prior to scheduled appointments

Program Support and Development

• Support implementation and ongoing improvement of MAT program workflows and processes
• Assist in identifying opportunities to enhance patient engagement and program effectiveness
• Participate in quality improvement initiatives and team meetings

Community Outreach and Engagement

• Support outreach efforts to increase awareness of MAT services within the community
• Assist with development and distribution of program materials
• Build and maintain relationships with community partners and referral sources
• Represent LCH&D at community events and outreach activities as needed

Grant-Funded Program Responsibilities

• Support data collection and documentation to meet grant requirements
• Assist in tracking program outcomes, including patient engagement and retention
• Ensure compliance with grant-related expectations and reporting activities

They provide behavioral health evaluation and counseling to primary care patients referred by Lorain County Health & Dentistry medical and dental providers. Working closely with medical providers, they also assess and provide short-term intervention to patients during or shortly after medical visits. The majority of care provided by Licensed Social Workers and Licensed Professional Counselors is brief and solution focused.

ESSENTIAL FUNCTIONS

· Evaluate and assess the mental health and substance use related symptoms of medical patients.

· Intervene in crisis situations during business hours.

· Provide brief assessments to clarify behavioral health diagnoses and to inform care plans.

· Define targeted treatment goals with and for the patients

· Provide short-term counseling to an established schedule of Lorain County Health & Dentistry patients, which may also include family members.

· Identify patients with more intensive behavioral health needs and assist these patient in connecting to appropriate care within the community.

· Ensure all plans and strategies developed are personalized, age specific, and culturally appropriate.

· Provide education, counseling, and support to patients using trauma informed and research based interventions whenever possible.

· Develop positive, professional relationships with patients.

· Promote good health throughout the facility in conversation and behavior.

· Accurately and concisely document all patient contact on a timely basis in medical record.

· Collaborate with medical and psychiatric providers to ensure integrated care.

· Participate in a Peer Review Process to ensure quality and consistency of BH service delivery

· Comply with HIPAA and 42 CFR guidelines as all times.

They specialize in providing care for the whole family and as such deliver comprehensive, ongoing care to patients regardless of age or gender. As primary care physicians, they examine and treat adults and children alike with a great variety of conditions and refer those with serious ailments to a specialist as needed. At Lorain County Health & Dentistry, Family Practice Physicians are expected to develop long term relationships with their patients and their patient’s caregivers, provide patient informed individualized healthcare recommendations and plans and offer ongoing support and advice as needed in accordance with Lorain County Health & Dentistry’s philosophy of “partnership” with patients while integrating its values of Respect, Quality, Compassion and Hope.

• Educate patients and their caregivers on recommended techniques to improve and maintain good health.
• Promote good health throughout the facility in conversation and behavior.
• Refer patients to specialty care if needed for complex medical or psychological conditions.
• Respond appropriately when on call rotation.
• Complete encounters per policy.
• Respond to requests within EHR i.e. PAQ, tasks, patient message.
• Collaborate appropriately with provider team.

• Must maintain infection control standards at all times per company policy.
