Northwest Community Health Center (NWCHC) is seeking a professional, well-organized, team-oriented individual to fill its open Licensed Clinical Social Worker (LCSW) Position.
As a member of NWCHC’s multidisciplinary team, the LCSW has an opportunity to partner with other clinicians to encourage and help patients receive the behavioral support necessary to impact their overall health.
The LCSW will be responsible for providing brief intervention and consultation in real time to patients in a primary care setting; providing crisis intervention as needed; as well as, demonstrating knowledge of the principles of growth and development over the life span. The successful candidate will possess the skills necessary to manage patient assessment, treatment planning and episodes of brief goal focused therapy for established NWCHC patients; monitor mental health referrals to NWCHC’s for patients with higher behavioral health needs; coordinating care for complex patients with co-morbid mental and physical health conditions; functioning as a group visit team health educator; act as a consultant and resource to members of the care team on skillful and effective interventions with patients with chronic illnesses and mental health diagnoses; and meet minimum documentation requirements for all services.
The Northwest Community Health Center is a rural MT health center looking for a General Dentist to join its dental team. The selected candidate will be providing comprehensive oral health care within a patient centered medical home in collaboration with hygienists, one additional general dentist, and a part time pediatric dentist. The clinic offers preventive and restorative services to all age ranges, and conducts a significant amount of community outreach. Salary and benefits are very competitive.
Northwest Community Health Center (NWCHC) is looking for a full-time Advanced Practice Provider; Family Nurse Practitioner or Certified Physician Assistant, to join its multidisciplinary primary care team. NWCHC is an integrated primary care, behavioral health, pharmacy, and oral health delivery system in Libby and Troy Montana. The selected candidate will play a critical role in delivering a broad range of high-quality, compassionate primary care to patients of all ages including chronic disease management and follow up, acute care, as well as many in office procedures. He or she will be responsible for managing the needs and care of a distinct panel of patients in an outpatient setting.
All NWCHC staff participate in quality assurance projects and work among highly qualified professionals in setting goals and improving access to healthcare for all. Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
