We are looking for a certified Physician to join our expanding primary care/OB/GYN team and provide patients with high-quality treatment and diagnostic services. The person in the Physician role will be responsible for meeting, assessing and diagnosing patients in our clinic and providing them with healthcare services that meet state and national standards for caregiving. They will be responsible for prescribing patients with appropriate medication, health regimens and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues and/or promote a healthier life. The right candidate will have a passion for providing OB care along with their family medicine practice.

The person in the Physician/Pediatrician role will be responsible for meeting, assessing and diagnosing patients in our clinic and providing them with healthcare services that meet state and national standards for caregiving. They will be responsible for prescribing patients with appropriate medication, health regimens and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues and/or promote a healthier life. The right candidate will have a passion for providing diagnostic services to patients and will collaborate and work closely with other healthcare professionals in our organization.
RESPONSIBILITIES:

Patient Centered Care:
1. Facilitate health and disease patient education.
2. Support patient self-management of disease and behavior modification interventions.
3. Coordinate continuity of patient care with external healthcare organizations and facilities, including referrals to specialty care providers, and continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
4. Promote clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
Physical Requirements:
1. Must be able to lift 25 lbs.
2. Continuous sitting, standing, walking.
3. Correctable vision and hearing.
4. The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.

We need an Advanced Practitioner to work in our Women & Children Clinic. Family Practice focus with experience with Peds is desirable. Women's health experience is also desirable
Purpose of this Position: The Family Nurse Practitioner (FNP) is an interdependent member of a health care team and will function within established state and federal rules and nursing standards of care. The FNP will be required to exercise sound professional judgment while providing primary health care services to ambulatory patients within the specific population served by the clinic. Patient assessment, diagnosis and therapeutic measures will be carried out in close association with the clinic physicians, appropriate specialists and other care agencies in the community. The FNP will participate in staff meetings, conferences, in-services, training, continuing education and long and short term planning for Valley Family Health Care.

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories). Half-time service is also eligible at a reduced award. Eligibility extends to NPs, PAs, CNMs, CRNAs, and physicians with qualifying education debt.
