POSITION PURPOSE:
Responsible for health care coordination for patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Position will participate in QI activities as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill patients.
• Orders and interprets diagnostic and therapeutic tests relative to patient’s specific needs.
• Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
• Implements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role.
• Monitors the effectiveness of interventions.
• Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients.
• Collaborates with multidisciplinary team members by making appropriate referrals.
• Facilitates staff, patient and family decision making by providing educational tools.
• Other duties as assigned

JOB TITLE: Clinical Medical Assistant

POSITION PURPOSE
Performs Clinical duties as they pertain outpatient clinical care. Assists provider (Physician/Nurse Practitioner/Physician Assistant) with medical procedures and tests in conjunction with patient visits. Position will participate in QI activities as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Triage patient
· Escorts patients to exam room, and prepare patient for exam
· Obtains patient history and chief complaint
· Obtain lab specimens, give injections and performs routine medical tests
· Make referral appointments
· Provide patient education
· Call in prescription per PA/FNP/MD orders
· Performs light housekeeping chores
· Maintains adequate inventory of medication supplies
· Places pharmacy order
· File patient information in medical record
· Assist in assuring goals and objectives are met
· Receives telephone calls, answering inquiries, an/or refers questions to appropriate staff members
· Performs other related duties as assigned
Current CPR/BLS certification required.
Certification required to demonstrate competency in the performance of duties specific to infant, pediatric, adult and geriatric patients.
Must be knowledgeable of HIV, and Sexual Transmitted Diseases.
SKILLS/ABILITIES: Maintain Confidentiality; Good computer skills; Multi-tasking; Well organized with good oral and written communications skills.

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment (or up to $100,000 in some Substance Use Disorder Workforce categories).
