Primary Care Nurse Practitioner or Physician Assistant

Community Health is seeking a Nurse Practitioner or Physician Assistant to deliver primary care across its Rutland, Castleton, and Brandon clinics. This role involves providing comprehensive medical care, from diagnosis and treatment planning to patient education and emergency procedures. • Multi-site FQHC serving 80% of primary care needs in central Vermont.
• Certified Level III Medical Homes with panel management and case management systems.
• Robust integrated behavioral health program with embedded Psychiatrists, Behavioral Health Nurse Practitioners, and Licensed Clinical Social Workers.
• Fully integrated electronic medical record and practice management system (Medent) with robust interfaces with local hospitals.

Responsibilities

• Obtain complete medical history and physical examination on patients.
• Review medical records.
• Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures.
• Establish presumptive diagnosis.
• Order appropriate laboratory and diagnostic procedures.
• Develop a therapeutic plan for patients.
• Suture wounds.
• Prepare written prescription orders for drugs and controlled substances.
• Instruct patients and family regarding medications and treatment instructions.
• Provide recommendations regarding health and illness prevention.
• Recommend community resources to meet patient/family needs.
• Perform independent evaluations and treatment procedures in emergency situations.
• Provide monitoring and continuity of care between physician visits.

Requirements

• 3-5 years Primary Care Practice Experience.
• Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
• Nurse Practitioner or Physician Assistant license in the state of Vermont.
• Strong inpatient skill set.
• Quality-focused team player.
• Excellent organizational, time management and problem-solving skills.
• Ability to function effectively as part of a multi-disciplinary team.
• Effective oral and written communication skills.
