Physician Assistant - Family Practice

Position Title: Physician Assistant – Family Medicine / Primary Care

Primary Role: Provide comprehensive, accessible, and culturally responsive primary care services to patients across the lifespan, with an emphasis on preventive care, chronic disease management, health education, and care coordination.

Population Served: Diverse and medically underserved patients, including individuals and families who may experience barriers related to language, socioeconomic status, insurance coverage, transportation, and access to healthcare.

Language Requirement: Bilingual Spanish/English proficiency is required.

Primary Clinical Responsibilities:

Patient assessment and diagnosis; treatment of acute and chronic conditions; preventive healthcare; medication management; ordering and interpretation of diagnostic testing; patient education; referrals and care coordination; clinical documentation; and participation in quality improvement activities.

Supervision: The PA will practice in collaboration with the designated supervising physician and healthcare team in accordance with Illinois law, organizational policies, and the applicable practice agreement.
