Starting as a single clinic in 1971, Open Door now has 12 community health centers across Humboldt and Del Norte counties – a service area larger than the state of Connecticut – serving more than 55,000 patients a year and employing almost 600 members of the community.

The Position
Type of clinic/specialty: FQHC (HSPA PRIMARY CARE 19)
How many hours per week: 40
Whom else will the provider be working with?, is this a solo position: Must have 3 years experience! Candidate must be able to manage their own patient panel, with experience managing their own patient panel. However, they do have physician mentorship.

What are the typical work hours: 8a-5p (two 4-hour shifts per day)
How many patients per day: Minimum Goal - 9 patients per shift, 18 per 8 hour day

Compensation and benefits
Salary range: $148,627.20–$168,553.60
Production or quality incentive: Yes, there are incentives
INCLUDING BUT NOT LIMITED TO - providers hitting their expected 9 patients per shift (18 per full day) $12,400 Annually - IF they see 10-12 patients per shift (20 - 22 per full day) $44,000 Annually.
Sign on bonus: Up to $25K

PTO: A ccrual based upon years of service: 1-4.99 years = 15 days; 5-9.99 = 20 days, 10-14.99 = 25 days, greater than 15 = 30 days

INTERVIEW PROCESS:

2. Phone screen with Angela and Nathan - once they wrap up, goes into the system to #3
3. Teams/Zoom with Associate Medical Director - the AMD will reach out within 48-72 Hours
After this wraps up, goes into the system to #4 - They may skip #4 and go straight to #5
4. Teams/Zoom with VP of Health Services
5. Site visit - They cover all interview expenses including family

IF THE PROVIDER IS INVITED OUT FOR A SITE VISIT, THEY HAVE BASICALLY ALREADY DECIDED THEY ARE GOING TO MOVE FORWARD WITH AN OFFER. THE SITE VISIT IS A FORMALITY FOR SEEING THE SITES, DECIDING WHICH SITE IS BEST, WHERE THEIR CRITICAL NEED IS ETC.

CONTRACT PROCESS - offer within 42 hours

NHSC Loan Repayment: This position is at a National Health Service Corps (NHSC)-approved site. Base salary: $131,227.2 - $148,137.6/yr. Providers here may qualify for up to $75,000 in tax-free federal student-loan repayment over a 2-year full-time commitment — a total potential first-two-years package of up to $371,275.2 ($296,275.2 in base salary over two years + up to $75,000 in loan repayment). Award amounts depend on the annual application cycle and your eligibility.
