Sonoma Valley Community Health Center (SVCHC) is a non-profit, federally qualified health center (FQHC) that opened in 1992. SVCHC began as a grassroots effort to address the lack of access and disparities of health care for the uninsured and underserved in Sonoma Valley. Since its inception, we have grown in capacity to serve Medi-Cal, insured, underinsured, private insurance, private pay patients and provides sliding scale fees for those in need. In addition to medical services, we offer behavioral health, an in-house phlebotomy service, dental clinic, pharmacy services and certified bilingual enrollment professionals.
SVCHC is seeking a full-time Family Nurse Practitioner (FNP). Under general supervision and in accordance with standardized procedures, the FNP performs physical examinations and treats common episodic and chronic health care problems with an emphasis on health maintenance and disease prevention through education and counseling. The FNP works in collaboration with other health care team members, consulting and referring as appropriate, but retains responsibility for ongoing clinical management of the patient.

Federally Qualified Health Centers participate in the National Health Service Corps (NHSC) Loan Repayment Program. Clinicians who serve at least two years full-time at an FQHC can receive up to $75,000 in tax-free loan repayment. Half-time service is also eligible at a reduced award (up to $37,500). Eligibility extends to NPs, PAs, CNMs, and physicians with qualifying education debt.
What that's worth here: a provider carrying a $100,000 balance who takes this role and serves the initial two-year commitment would be debt-free in roughly 10 years — the NHSC award first, then PSLF forgiveness of the remainder (FQHCs are qualifying PSLF employers).
