Community Health Center is seeking a dedicated Primary Care Provider to deliver comprehensive medical services, primarily to underserved populations. This role offers the opportunity to work in an innovative practice environment with strong clinical support and Advanced Access scheduling. Requirements

• MD/DO from approved medical/osteopathy school and graduation from accredited residency program in family practice (Physician)
• Must be board-certified within 24 months of commencing employment at CHC (Physician)
• Hold license or be eligible for license in State of CT (Physician)
• Bi-lingual skills English/Spanish preferred (Physician, APRN, PA)
• Graduate of an accredited Master of Science in Nursing program with a specialty as a Family Nurse Practitioner (APRN)
• ANA certification as a Family Nurse Practitioner (APRN)
• State of Connecticut APRN and RN licensure (within 12 months) required (APRN)
• Medical - State of Connecticut Licensure (APRN)
• Graduate of an accredited Master program in the study of physician assistant (PA)
• Maintains appropriate board certification, licensure, and professional organizations (PA)
