Position Summary

ACCESS Family Care is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our mission-driven healthcare team. As a Federally Qualified Health Center (FQHC) and HRSA-approved site, ACCESS Family Care is committed to providing high-quality, comprehensive healthcare services to underserved populations throughout our communities.

The Family Nurse Practitioner will provide primary care services to patients across the lifespan, emphasizing preventive care, health promotion, disease management, and patient education. This position offers the opportunity to make a meaningful impact while working in a collaborative, patient-centered environment.

Essential Responsibilities

• Provide comprehensive primary care services to pediatric, adolescent, adult, and geriatric patients.
• Perform physical examinations, establish diagnoses, and develop treatment plans.
• Order, interpret, and communicate laboratory, imaging, and diagnostic test results.
• Prescribe medications and treatments according to state regulations and organizational policies.
• Manage acute and chronic health conditions.
• Provide health education and preventive care counseling.
• Coordinate care with physicians, specialists, behavioral health providers, and other healthcare team members.
• Maintain accurate and timely electronic medical records.
• Participate in quality improvement initiatives and meet productivity standards.
• Comply with HRSA, FQHC, and organizational regulatory requirements.

QualificationsRequired

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
• Current Nurse Practitioner licensure in Missouri (or ability to obtain).
• Current national certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
• Current DEA registration and Missouri controlled substance registration (or ability to obtain).
• Current BLS certification.
• Strong communication and patient relationship skills.

Preferred

• Previous experience in family practice, community health, rural health, or FQHC settings.
• Bilingual skills are a plus.
• Experience working with underserved populations.
