Reporting to the Chief Operating Officer, the BH Program Director will provide the leadership needed to effectively manage and grow the Behavioral Health Service Line by ensuring clinical excellence, operating efficiency, and financial strength. This will include direct oversight of all Outpatient Behavioral Health services, including oversight of School-Based Health Centers. The Director will work collaboratively with medical leadership to integrate behavioral health and medical services. This role will also be the liaison for all federal, state, and local mental health departments and will stay abreast of changes occurring at those levels by attending regular meetings and building relationships with and establishing contacts within these external agencies.

Job Summary: Responsible for providing coordinated and quality primary care services to patients utilizing the clinical services of Community Health & Wellness Center (CHWC). Clinical services provided shall include routine and appropriate ambulatory adult, pediatric, and gynecological care as assigned. The Physician will be knowledgeable about agency and clinical policies, keep all patient information confidential, and shall assume responsibility for the development and realization of an individual ongoing care plan for all patients including the performance of selected procedures.
Qualifications:
M.D./D.O. degree from an accredited school. Documented completion of a residency from an accredited institution in an appropriate specialty. Board certification or Board eligibility desirable. Current licensure in State of Connecticut. Valid Federal and State Controlled Substance Licenses.
Requirements: Experience in primary care setting preferred. Ability to work as part of a multidisciplinary team. Interest in serving low income-multicultural populations. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Demonstrates knowledge of the principles of growth and development over the life span. Ability to identify and categorize each patient’s age specific grouping of needs, such as those for infant, adolescent or geriatric patients. The ability to establish and maintain collegial relationships with coworkers, and to function in a calm manner during stressful and emergency situations. Duties and Responsibilities:
1. Assures provision of primary health care to all Community Health & Wellness Center patients including but not limited to:
a. Establishing a data base on patients by obtaining an appropriate health, developmental and psychosocial history, as well as thoroughly conducting a physical examination and ordering/performing and interpreting screening and diagnostic procedures and tests.
b. Providing treatment according to accepted agency standards and protocols.
c. Assisting in the management of referred and/or complex problems with other members of the agency and community health nurses, mental health workers, social workers, and other community resources.
d. Performs minor office procedures, dressing changes, and ambulatory treatments and diagnostics in accordance with approved privileges.
e. Works in a collaborative and appropriate manner with other agency providers, clinical support staff, and administrative personnel.
f. Recognizes and utilizes opportunities for health education for patients and families.
g. Maintains accurate legible medical records and returns all assigned charts or patient data (labs, reports, forms, etc.) to Medical Records in a timely fashion (i.e., charts before clinics begin the following day and other data within two days). Enters data into electronic medical record in a timely fashion as per CHWC policies.
2. Demonstrates efforts to maintain and improve job specific competencies and incorporates new knowledge into practice.
3. Acts in accord with standing agency contractual agreements with health maintenance organizations, insurance plans, and other managed care organizations.
4. Participates in agency quality assurance and improvement efforts.
5. Works jointly, as requested, with the Medical Director and/or other administrative personnel in the areas of utilization management, data retrieval, and the establishment and/or revision of clinical policies and protocols.
6. Acts as a resource to all Community Health & Wellness Center sites when requested. Participates in rotation to provide services to the community in the CHWC mobile van as needed.
7. Maintains ongoing education through completion of required continuing medical education credits.
8. Demonstrates knowledge and application of principles of fire safety, infection control, hazardous communication program, universal precautions and general safety.
9. Assists other staff, when time permits, in the completion of their duties to assure smooth function of the office. Assists as requested in ongoing education efforts for on-site medical, nurse practitioner, and physician assistant students.

Essential Functions:

Position Summary:
This position will play a key role providing quality dental care in our fast-growing primary care facility. The prime focus will be examining, diagnosing, and developing treatment plans for infectious and injuries of the teeth and mouth. Carrying out the treatment plan in a competent and efficient manner is then expected.

•Proficient in preforming dental procedures including: examinations and related diagnoses, amalgam and composite restorations, root canal endodontic therapy, extraction of erupted teeth and roots, simple surgical procedures, and denture fabrication.

• Exercises direct supervision of the assigned/designated dental assistants.

Position Summary:
The Dental Assistant is responsible for supporting the dentist and dental hygienist in providing care to the patients of the health center. The Dental Assistant is responsible for chairside assisting, preparing and cleaning rooms, inventory control, and sterilization. Dental assistant" means a person who: (A) Has (i) completed on-the-job training in dental assisting under direct supervision, (ii) successfully completed a dental assistant education program accredited by the American Dental Association's Commission on Dental Accreditation, or (iii) successfully completed a dental assistant education program that is accredited or recognized by the New England Association of Schools and Colleges; and (B) meets any requirements established by the Commissioner of Public Health (Conn.
