Our FQHC LAL is looking for a General Dentist that is compassionate and loves to help patients. Our main focus is to provide proper care of the patient's needs.

MINIMUM REQUIREMENTS:

• Licensed as a Dentist in the State of California
• Knowledge and skills in delivering dental care to patients
• Good communication skills

PRIMARY RESPONSIBILITIES:

• Maintain the dental health of our patients and their families
• Examine, diagnose and treat diseases, abnormalities, injuries and functional disorders of the oral cavity
• Evaluate findings and develop treatment plans for patients.
• Perform routine preventive and periodontal treatment as medically necessary

TITLE: FAMILY NURSE PRACTITIONER

REPORTS TO: DIRECTOR OF PRIMARY CARE & QUALITY

STATUS: EXEMPT

LOCATION: ANTIOCH, CA

PAY RANGE: 125- 35K per Year plus a sign on bonus

POSITION SUMMARY

The Family Nurse Practitioner (FNP) will be responsible for direct patient care and for other administrative duties as designated by the Director of Primary Care & Quality. The Medical Doctor will provide the overall direction for all patient care activities and the Director of Primary Care & Quality will provide you with administrative supervision.

RESPONSIBILITIES:

Primary Job Functions

1. Primary Care Provider providing complete patient history, physical examinations, diagnosis, and plan for treatment of primary, secondary, and tertiary care services as outlined in, and in compliance with the standardized protocols for Nurse Practitioners.
2. Primary care delivery includes communicating with patients or guardians of patients regarding care plans, results of diagnostic studies, and answering patient and family questions. Also included is organizing and delivering health education for patients, including preventative health information and disease specific education. The primary care provider coordinates patient referrals for specialized health care to public health care agencies.
3. Reviews patient file/record, including allergies, problems, medications and immunization status.
4. Orders or executes various tests, analyses and diagnostic images to provide information on a patient's condition.
5. Analyzes reports and findings of tests and examination and diagnoses the condition of patients.
6. Administers or prescribes treatments.
7. Determines and prescribes medication, dosage and schedule given the patient’s condition and allergies.
8. Prescribes vaccinations to immunize patients from communicable diseases.
9. Provides prenatal care to pregnant women. Provides postnatal care to mothers and infants.
10. Perform the care services listed below (or be willing to learn those services). Services may include:
11. Minor surgery (toenail removal, mole and cyst removal, foreign object removal, etc.)

21. Communicate with the Medical Doctor regarding quality care and treatment issues of patients at BBFHC.
22. Conduct Clinical Pertinence Chart Review studies in order to assess and enhance patient outcomes.
23. Serves as a resource for community agencies.
24. Use technology tools for research, data entry, record keeping, and business communication via email.
25. Meet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc.
26. Follow all internal policies and procedures; be accountable for completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation.
27. See patients in a timely and efficient manner, stay reasonably on time for scheduled appointments in family practice throughout the day, unless exceptional circumstances occur.
28. The financial viability of Brighter Beginnings requires that all providers be held accountable for their actions with regard to proper coding and appropriate charging for all services rendered.
29. Charting in the patient’s medical record is routinely to be completed within 48 hours of the patient’s visit. Establish and maintain a confidential medical record for each patient.
30. Communicate with the Director of Primary Care & Quality, reporting on issues related to the functioning of the BBFHC, and the Director of Primary Care & Quality reports to the CEO and partner agencies of BBFHC.

Administrative Job Functions

1. Demonstrate on-going support and respect for administration in all communications, either privately with staff or in a group setting.
2. Address any administrative issues directly and in private with those concerned.
3. Time management: routinely see patients in the time allotted, adjusting to daily patient flow issues to remain on schedule.
4. Work productivity must demonstrate ability to see a minimum caseload of 18-20 patients per day (8 hour shift) and 23-27 patients per day (10 hour shift); effectively work patients into their schedules as caseload demands and respond to a variety of unforeseen daily circumstances, including emergency care of patients.
5. Reporting: FNP’s report directly to the Medical Doctor and his/her designees, which include the Chief Executive Officer.
6. Complete patients charts and submit medical bills daily.
7. Must be flexible, adapt to change, and serve as a role model for staff through interactions; should at times think outside the box, providing possible resolutions/solutions to issues/concerns, rather than simply pointing out a problem area;
8. Comply with all HIPAA rules and regulations.

POSITION SUMMARY

The Family Practice Physician (Physician/MD/OD), provides primary and continuing medical care for patients seen at the Brighter Beginnings Family Health Clinics (BBFHC). Physicians take account of physical, emotional and social factors when diagnosing illness and recommending the required treatment. Patients may be referred to hospital clinics for further assessment and possibly for treatment, working with the Brighter Beginnings Family Health Clinic providers to ensure quality outcomes.

RESPONSIBILITIES:

ESSENTIAL DUTIES AND RESPONSIBILITIES OF FAMILY PRACTICE PHYSICIAN:

Typical work activities include:

· Respond to medical/health problems presented by patients including history taking, diagnosis, investigation, treatment and referral to secondary and tertiary care as appropriate;

· Maintain patient confidentiality in accordance with HIPAA regulations.

· Organize preventative medical programs for individual patients;

· Provide specialist clinics for specific conditions or for certain groups, e.g. diabetes, pregnancy and post-partum, pediatrics;

· Meet targets set by federal agencies for specific treatments, such as child immunizations;

· Use Electronic Health Records to keep track of patient visits, billing, and clinical outcomes;

· Keep up to date with medical developments, new drugs, treatments and medications, including complementary medicine;

· Observe and assess the work of other providers in the clinic.

· Participate in quarterly clinical chart review studies in order to enhance patient outcomes.

· Review any medical records as requested by providers for quality care and treatment issues.

· Use Electronic Health Records to keep track of patient visits, billing and clinical outcomes;

· Keep up to date with medical developments, new drugs, treatments and medications, including complementary medicine;

· Observe and assess the work of other providers in the clinic.

· Participate in quarterly clinical chart review studies in order to enhance patient outcomes.

· Review any medical records as requested by providers for quality care and treatment issues.

· Maintain a portfolio of continuing professional development (CPD) activities.

· Complete medical charts and bills timely.

· Provide indirect patient care through collaboration with providers by phone or in person and be available for consultation by electronic communication at all times.

· Assist in reviewing on all practice protocols for nurse practitioners and clinic policies and procedures.

· Participate in quarterly clinical chart review studies in order to enhance patient outcomes.

· Review any medical records as requested by providers for quality care and treatment issues.

· Will report concerns regarding quality assurance or practice issues to CEO and Clinical Manager.

· In coordination with the health management team, initiate and develop special clinics and programs to meet the needs of the community including diabetes, asthma or other chronic disease management programs.

· Establish and maintain understanding and cooperation with local organizations toward effective community health.

· Provide direct leadership for Quality Assurance, Utilization Review and Medical Advisory Committees.
