Position Summary

The provider will be a fully licensed and accredited Advanced Practice Provider (APP) authorized to practice in the designated state, with board certification in Family Practice preferred. This role is responsible for delivering high-quality, patient-centered primary care within Health West’s designated service areas.

The provider works collaboratively with the Director of Advanced Practice Providers and the Chief Medical Officer, who offer guidance, support, and performance evaluation. This position involves potential exposure to bloodborne pathogens due to direct patient care, trauma response, and laboratory procedures.

Schedule

We offer flexible scheduling options to support work-life balance:

• 32-hour work week (Monday–Thursday), or
• 40-hour work week (Monday–Friday), with Fridays spent in Quick Care

Detailed Responsibilities

• Provide direct primary medical care in a primary care clinic setting
• Counsel and advise patients and their families on treatment, care, and lifestyle modifications
• Refer patients to appropriate ancillary and supportive services as needed
• Participate in after-hours, on-call coverage of clinics as assigned
• Serve as a member of the Quality Assurance (QA) committee when assigned
• Maintain strict confidentiality of all patient and organizational information
• Supervise, mentor, and evaluate mid-level providers as assigned
• Perform additional duties or assignments as outlined in individual employment agreements
